[Question #11574] Hiv/stds mam

So i Have a few encounters, I haven’t done these not being females in about a year, and use condoms, so always test negative, and don’t worry much. But this time feels different. So first endounter 17,18 days ago(2 1/2 weeks) i had sex with a trans(man) I got unprodected oral for about 15,20 minutes, and about 20 minutes pf penetrative sex with a condom. My worry is that during the enounter , I also grinded on his ass for about 2 minutes.(no condom) I am 98% no penetration there, but I am terrified of the two percent accidentally slid inside without the condom. Also we did super intense makeout about 10 minutes, and I have two holes in my teeth, and did a bit of frottage, so terrified this fould have gotten me hiv. Also during the encounter I used a condom, took it off , and put on another one.so a few days later, maybe like 5,6 I started to notice some weird white stuff, no bumps or burning around genitals. I actually wemt to get a ghonrrhea and clahymidia test two days ago, and waiting on results, I wasn’t even worried about hiv, since I used a condom. But that being said fast forward today 2 1/2 weeks later I started to get a sore throat and a runny nose, thought ok maybe it can be some allergies, but now today I woke up And have a horrible sore throat, and I have white stuff on my throat also, and I woke up with chills, and I never get chills, not even when I had covid, and almost never get fevers, and now I feel I am developing a fever, even my mom who is a nurse, said I was super hot. my second Head from a trans about two weeks ago, even after white stuff on my penis. The third encounter was unprotected oral from trans, and protected anal, no making out or nothing. I usually avoid these things(msm) due to being terrified of hiv, but unfortunately have been wanting some lately.l, so here goemy questions.

I know this is considered my first follow up question and I am ok with that.
2) apart from herpes while I know oral stds are technically possible, what is the realistic chance of getting ghono, chalymidia, sip, from Just receiving unprotected oral? If you were to put a percentage on it.
3) during first encounter I used a condom, took it off, to receive unprotected oral, may have touched feces, because I did finger her also, and when I put on another condom, I put my finger through it, to make sure which is the correct way, could I have gotten hiv like that from touching feces, and basically putting on another condom.
4) should i take prep for my 3rd encounter msm, 2 days ago, unprotected oral, and protected anal.
5) do
My symptoms sound like ars, also feels like lymph nodes are a bit swollen, extremely sore throat, with white stuff, and my chills and I never get them , even when ai had covid no chills.and past few days had some slight redness on my cheeks, and I never get that
6) hiv or stds from frottage?

You've describe a lot of details about your recent sexual exposures that make little or no difference. Although you chose a partner likely at high risk (if I understand correctly, trans M to F, still anatomically male). Therefore your insertive condom-protected sex probably was anal, correct? The other details -- frottage, your teeth, etc don't make much difference. Did you ask your partner about her HIV status? That always should be a component of safe sex. If she doesn't have HIV, or is infected but on effective anti-HIV treatment, there was no HIV risk even without condoms. And since penetration was condom protected, the risk of other STIs also was very low.
As for your symptoms, they came on much too late to be caused by acute retroviral syndrome (ARS, i.e. new HIV infection); and anyway ARS doesn't cause nasal congestion or runny nose; and the sore throat, if present, usually is quite mild. Almost certainly you caught a cold, that's all.
To your specific questions:
2) Chlamydia rarely infects the throat, and when it does, it not easily transmitted. So urethral (penile) chlamydia is zero risk from oral sex. Gonorrhea risk is higher, but no numerical estimate is possible. Probably up to 5% of people like your partner have pharyngeal (oral) gonorrhea; and if present, it's probably transmitted maybe around 10% of the time. Combining those estimates means well under 1% chance. And if you had acquired gonorrhea, you would have had pus dripping from your penis within 5 days. So you definitely can expect your gon/chl test result to be negative. Syphilis is even less likely; the condom-protected anal sex probably was higher risk than the unprotected oral. In any case, absence of a chancre (the sore of primary syphilis) by ~3 weeks is itself good evidence you weren't infected.
3) These are the sort of details I referred to in my opening comments. They do not materially increase your risk of any STI.
4) You mean PEP (post-exposure prophylaxis), not PrEP. All PEP decisions should be made with local health care provider. I would say your HIV risk is low enough that PEP isn't warranted. HIV is zero risk from oral sex (there's never been a proved case of HIV transmitted oral to penis), and of course near zero for condom protected anal.
5) Your symptoms are not suggestive of ARS, as discussed above.
6) Also addressed above. Theoretical risks for herpes, syphilis and HPV, but low risk for all of these.
I hope these comments are helpful. Let me know if anyting isn't clear.

Hey just to clarify a few things yes both encounters were msm(trans) and involved protected anal sex , I believe I did a typo. But I am back because I was super worried about chlaymidia / ghono, but both results came back negative 2 weeks after the exposure I was worried about. Now I am a bit nervous Not sure if ai mentioned, One of the reasons I started to worry was I encountered whiteness on the back side of the penis, under the head. It does not burn or itch, or have any bumps, but there is clear whiteness looks almost shiny, no odor( I am not circumcised. I do not believe it is smegma, and everything I see online seems to point to a yeast infection. I have also been hooking up unprotected with a female companion. But we both got tested 5 months ago, and everything came back negative for both of us.
1) what do you believe this white stuff could be? Could she have give me a yeast infection? Could I have gotten a yeast infection from frottage?
2) chlaymidia/ghono tests taken 2 weeks post exposure could I have received false negatives?
3) could I be dealing with herpes? The whote stuff has been there for over two weeks, and No crusting, burning itching
4) I read online that if it is a yeast infection, for me one of the biggest reasons would be hiv, so that has triggered my hiv thoughts again, that I have thrush, I do not have diabetes, got tested for everything 4 months ago. So seems is a possibility, do you think that is a possibility with my encounters?
5)Also I wanna add I always check my penis alot, due to amount of sex I have bad throughtout my life, and have never seen that white stuff, until a week after the encounter with the trans I had on July 18, the timing just seems way to coincidental, and I still have it on my penis, any insight on what it could be ?

1) If you are uncircumcised and the white material is under the foreskin, it sounds like normal smegma. It's not yeast or other infection if not inflamed (itchy, irritated, reddened). No STD does this.
2) The negative results are reliable; false negatives are rare.
3) Your description doesn't suggest herpes.
4) Wrong about HIV and yeast. Genital yeast infections amost never are due to HIV or any other immune deficiency. And anyway it's too soon for you to be immune deficient from a new HIV infection.
5) No comment. It has nothing to do with the sexual exposure described. Just retract your foreskin and wash with soap and water -- which all uncircumcised men should do once a day anyway.

Hello Doc thanks for all the answers, I have a few more things I would like to ask, it may help me with future encounters etc, or others, and maybe I can have peace of mind, whenI need it by returning here. But I have a. Question about condoms, why is there so much different information everywhere about the effectiveness of condoms, it seems to range from 60-95% on many different sites for hiv.
1) can you give me your stance or a high detail of if you think it is possible to aquire hiv from using a condom, 100% proper use, and no breakage?
2) if I told u I just had sex with some one(male to male) a day ago, and they told me they were confirmed positive of hiv, whithin only 12 hours, and had a high viral load, but we made out, they gave me unprotected oral, but I used a condom the proper way with no breakage as the giver of anal sex, would you as my doctor recommend me take pep? Or would I be safe from hiv?
3. Would you recommend prep or pep demand for some one that does msm only once or twice a month,? Can you tell me more of prep on demand, don’t seem to see much info about it anywhere, seems many people don’t know about it?
4) can u tell me more about doxyprep, who would you recommend that to as well? Is it worth taking etc

The opportunity for follow-up comments is for clarification of the original question -- not to raise entirely new issues. In addition, we're not inclined to respond to hypotheticals. You should be able to find our views on all four of these issues by searching existing threads.
Best wishes and stay safe.
