[Question #11593] Hiv and std risks

I had protected vaginal sex with a CSW in china, health condition unknown, for 10 sec. French kiss 2 mins & finger inserted for 2mins.I have 2 fresh mouth ulcers. No cuts and bleeding on the finger but had weird feeling on the tip of the nailbed afterwards, Condom were intact and complete. slid off with tissue paper from penis then filled with water & no leakage. I was drunk, room was dark so I was not able to exam CSWs health condition.


Questions:1. What’s my hiv risks? 2. What’s my risk for other STDs, 3 Do I need PrEP for the exposure

Welcome to the forum. Thank you for your confidence in our services.

This was a very low risk exposure. The vaginal sex exposure was brief and condom protected, and kissing and fingering are no risk. It doesn't matter whether or not you could "examine the CSW's health condition." That's useless for HIV/STD prevention anyway. (The condom water test also was necessary. There is no such thing as microscopic leaks that allow bacteria/virus exposure. If a condom doesn't break open, protection is complete.) To your questions:

1. No risk at all for HIV. 2. No risk for other STDs. 3. You should not seek PrEP in this situation. Probably you would not be able to find any doctor or clinic willing to prescribe it, assuming you were honest about the sexual exposure. You don't even need testing for HIV or any other STD. Please don't worry.

Just further to my questions.

1. You said the water test was necessary, does that mean as long as the condom can hold water & no leakage, then the protection is complete? when condom breaks, it will break like a ballon and wild open?

2.  I'm actually more concern about STDs, esp HSV2, HPV and syphilis. there are quite a lot contradictive says about whether or not the condom will give you full protection against the above virus (the area that a condom cannot cover?, say the bottom shaft/part of the penis or testicle bag might have been exposed to CSW's vaginal fluid etc, and it was dark so I couldn't tell if theres any obvious sore also makes me worrying.

3. I understand in medical there's no such thing of 100% and this question might be repetitive and irrelevant. JUST FOR THE SAKE OF SPECULATION !! Assuming the CSW I had in China was infected with everything (HIV + all sorts of STDs). + some of my bottom penis wasn't fully covered by condom, + my fresh mouth ulcers + finger inserted (no cuts as I put hand sanitizer afterward no pain or sting). Would that change your opinion?

1. Correct; you understand. If a condom does not break wide open during sex, protection was complete.

2. It is normal for the skin above a condom to be exposed. This is why condoms work somewhat less well against STDs transmitted by skin-to-skin contact (HSV, HPV, syphilis) than those transmitted by fluids (HIV, gonorrhea, chlamydia). However, for any single exposure, a properly used condom that does not break is excellent protection against all STDs. Don't believe everything you read online -- I suggest you limit your online investigations to professionally run or professionally moderated sites, and avoid those by and for people at risk (like Reddit, for example).

3. No. those circumstances would not change my opinion or advice. For those reasons, from a medical/risk standpoint, you to not need testing for anything. However, you are free to test if you would like the additional reassurance of negative test results, in addition to my reasoned, science-based reassurance. It's up to you.
