[Question #11603] Oral Scare HIV / STI's

Dear Doctors, it is indeed a pleasure to interact with esteemed professionals as yourselves.
I'm a 42yr old Heterosexual male.
On the 23rd of June, I received un-protected oral from a young lady at a Massage Parlour, during the encounter she asked me to dry hump her from behind where my penis may have touched her naked anus / vagina from the outside, (No vaginal or Anal penetration occurred) lasted a few seconds as I don't particularly enjoy the act. After which she continued oral and I finished in her hands as she does not allow ejaculation in her mouth.
A few days later out of anxiety I visited her again to confirm her sexual health, she assured me that she was clean and even showed me a HIV test report done in Dec 2023, which was Negative. She explained that she only gives BJ's to people she is comfortable with and has sexual intercourse only with her BF and no clients at the Parlour.
Out of fear of HIV I took an RT PCR HIV test on the 6th of July, 13 day post exposure, which came back negative.
Today 18 days post exposure I have a red rash in my right armpit, I have had no other systems what so ever in connection to Gonorrhoea / Chlamydia (Discharge / Painful urination ).
Questions:
Could this be a symptom of HIV, I'm extremely anxious, could I be infected ?
Also, has a negative PCR on 13th day ever returned a positive 4th gen test ?

Welcome to our Forum. Thanks for your questions. I'll be glad to comment. The exposure that you describe did not put you at any risk for HIV. There has never been a proven case of HIV acquired from receipt of oral sex and penetration is required to be at risk for HIV through exposures at other sites. Contact with oral or genital secretions outside of the body as might occur with the rubbing you describe or due to receipt of masturbation are no risk activities. Your HIV PCR confirms that you did not acquire HIV as results of PCR tests are conclusive any time more than11-12 days after an exposure. You did not acquire HIV. The rash you noted is not due to HIV. Thus, in reply to your specific questions:
Could this be a symptom of HIV, I'm extremely anxious, could I be infected ?
Please don't worry. Your rash is not due to HIV or any other STI
Also, has a negative PCR on 13th day ever returned a positive 4th gen test ?
No, never. Your results are conclusive

Appreciate your quick reply and am feeling better about this entire episode.
Quick follow up question, after seeing the rash I took a 4th gen test and another RT PCR today, 18 days post exposure, the 4th gen came back non reactive, PCR will be available on Saturday. Hypothetically speaking if the rash was indeed due to HIV the negative 4th gen would not be possible. I had read in the forum either yourself or Dr. HHH had mentioned, that a negative 4th gen would not be possible if they were symptoms from HIV. Please correct me if I've misunderstood the information.
Do I need to test any further or can I put this behind me with the RT PCR result on the 13 lth day

Correct- if your rash were due to HIV both your 4th generation test and your PCR test would have been positive. That they were not proves that HIV did not cause your rash
The exposure you describe was a no risk event. There was really no scientific reason for testing at all.

Quick update, the PCR test for HIV came back Not-Detected, 18 days post exposure, this is definitely conclusive, yes? Along with the previous PCR's negative on 13 days post exposure ?

Dear Dr. Hook,
Apologies in advance to bother you, but I have a small doubt.
You had mentioned that the results from both the PCR tests could be treated as conclusive, is this based on the no risk encounter I had or on the PCR tests themselves. I had read in the forum Dr. HHH mentioning that a PCR test can't be conclusive on its own and needs a 4 th gen on 28 days to be considered conclusive. I do believe I am over thinking and being unnecessarily anxious, but your reassurance will mean a great deal in moving forward.

Final response. While Dr. Handsfield and I both previously suggested following up PCR tests for HIV with a fourth generation combination HIV antigen/antibody test at 28 days or beyond, accumulating evidence has provided assurance that results of HIV test from specimens taken anytime more than 11 days following in encounter can be considered conclusive. We have never seen a HIVPCR test taken beyond 11 days go to later become positive. Please don’t worry.
