Dear Doctor,
I hope you are doing well. I have some doubts regarding the symptoms i am experiencing now. I had a protected blowjob from an escort 38 days ago. and i did not even let it finish because i immediately left. my concern is that all of sudden i looked at te condom and there was some liquid inside but not that much just the bubble tip. so i got concerned maybe the escort used a previously used condom and it was not my cum because i was not really conscious. then i asked her whether u used a new one or old one that she said "obviously new one". i got sick 5 and 6 days later with no fever and just a small sore throat but now after 38 days i am experiencing a high fever (38.4), sore throat, and pain in my neck and a little in my legs.
1- do you think it is ARS? because i read your previous messages and most of them stating that it is too late for ARS after 38 days. can you tell me your opinion about this?
2- do i need testing? although i know it is paranoia and not possible to get infected by this way
3- can i stay relaxed and not avoid my girlfriend?

Welcome to our forum. Thank you for your questions. I’ll be glad to comment. Almost certainly a small amount of liquid that you saw at the tip of the condom was what is called pre-ejaculate which represents small amounts of fluid which are released during intercourse prior to ejaculation. This is normal. With regard to your specific questions:
1. Neither your sore throat at five days or your current symptoms are the ARS. An isolated sore throat at five days is not characteristic of the ARS syndrome and occurred too soon. On the other hand, the ARS does not occur more than 30 days following acquisition of infection either. Thus, your timeframe for the onset of symptoms is not consistent with the ARS. it is far more likely that you have influenza, COVID-19, or one of the every day viruses most of us get from time to time
2. Testing is always a personal choice however, I see no medical reason for testing at this time.
3. As I said above, I see no reason for concern. Thus I also see no reason to abstain from unprotected sex with your regular partner.

I appreciate your help and response dear Dr. I have another question as well. i touched the vagina of the person with my penis (with the shaft and almost covered), then the person gives me a blowjob (protected) and i however touched my penis with the used condom at the end when i was removing it (no vaginal or anal penetration at all and only blowjob),
1- so is it possible that her saliva passes the hiv? (i know saliva kills HIV although)
2- do you consider this as zero risk for hiv?

No, saliva does not transmit HIV, particularly when transferred indirectly in the manner you describe
