[Question #11699] Hiv Risk - Unprotected Vaginal Sex

Doctors,

I'm an uncircumcised 37 y/o heterosexual male living in the southwest. In May, I met a 28 y/o Hispanic female at work (construction). We had brief unprotected vaginal sex which lasted 2–3 minutes and I finished in her, She told me she had an IUD. I asked about her status, She said she had been tested last December after she found out her then BF cheated on her. She had her IUD taken out a week after we had sex and was tested again by her gynecologist. We had sex again on the 14th of this month, which was more of dipping (inserting and stopping) until I finished outside and then again on the 16th, which was prolonged in multiple positions, lasting about 10-15 min. What's my risk from this situation? She said I'm the only one she's had sex with this year, but she mentioned last year she had sex with 6 other guys, which makes me nervous, knowing she is willing to have unprotected sex with me without knowing my status. She's from a boarder town in TX which seems to have a low prevalence according to online research.

Welcome to the Forum and thanks for your questions. I’ll be glad to comment.

I would classify your partner as,low,risk.  She had negative tests in December and perhaps subsequently.  This and her forthright willingness to tell you about her past testing suggest that she is forthright about past infections.  

I would suggest that the need for testing is equivocal.  Your post suggests that this may be an ongoing relationship.  If so, I’d suggest that you suggest that you both go for testing together.  I would anticipate that your tests will be negative but that will allow you to move forward without con.

It is not a relationship I was concerned because I am not entirely sure I can believe what people tell me. I know single encounters are rare for transmission if a woman is infected, this would put me at a higher risk i think.

You can call it a relationship or not.  You describe sex with a relatively new partner at least 3 times.  If history predicts the future, you’ll have sex with her again.    My recommendations are unchanged.  EWH---

I’ve read that it is rare for a woman to have HIV in the USA, I also read HIV is more common in the south her being from TX will this increase the likelihood she has it? If she was to be infected given the 3 unprotected exposures I had what would be the chances I caught it? I’m almost at 3 weeks after the last event I have been having night sweats only in my neck area that causes my pillow to get wet (which could be from the heat living in the desert). I also had a small rash on my arm last weekend where the arm folds which lasted 3 days. Are these of any concern that could be related to HIV? I’m going to test for gonorrhea and chlamydia meanwhile even though i have no symptoms.

As you know, we provide up to three responses to each clients question. Therefore, this will be my final response, and the thread will be closed after that.

While Texas has relatively high rates of HIV infection for the United States, less than one in 1000 women in Texas have HIV ( i.e. 99.9% of women in HIV do not have HIV.

Neither your night sweats, nor the rash that you describe are in any way suggestive of HIV infection.  My advice remains the same. There is virtually no reason for concern.  No change to my suggestions or recommendations.
