[Question #11702] Worried about massage parlor

Hi Doctors. Would appreciate your advice (and possibly reassurance) as am feeling very stressed about a recent incident.
I went for a massage at a place that provides extra services. I have seen this same Thai masseuse before over the last few years every now and again. Normally it has been a straightforward happy ending with her topless. This time though, she also removed her trousers so was topless and wearing only lace/lingerie type underwear. She rubbed her body up and down against mine briefly. After that I had massaged/licked her breasts while she masturbated me with one hand and herself with the other.

What has panicked me is that afterwards I noticed under her underwear she had a sanitary pad and was therefore on her period. I was really worried that while she had rubbed her body against mine some blood may have gotten onto my body or penis. I am pretty sure she never used the same hand to masturbate myself and me, but am doubting myslf now. 

I am trying to reassure myself that as there was no oral sex or any penetration there is nothing to worry about. Also I think I read before that HIV does not survive outside the body so without actual penetration you are not at risk? But still feeling quite panicked. 

Also trying to reassure myself that it is a relatively 'high end' place and that the masseuse has been in the country for about 8 years. In the UAE this means she would have had mandatory HIV testing at least every 2-3 years in this time. 

Is there any reason to worry?? Or is this something to forget about? I found a clinic that does an HIV PCR test but think I need to wait 10 days for this to be meaningful?? Is there any reason to test in your view??? At 7 or 10 days how accurate would it be?

Welcome to the Forum.  Thanks for your questions.  Congratulation on your commitment to safe sex.  This encounter was safe with no risk to you for HIV or any other STI.  

The activities you describe - frottage (body rubbing/slide), receipt of masturbation and a lack of penetration makes this encounter no risk.  Even if your partner got some of her genital secretions or blood on the hand that she then used to masturbate you or her secretions or menstrual blood got directly on you, without penetration this would be a no risk event.  HIV does not pass through skin unless it is in the context of penetrative sex; the presence of any cut or scratches would not change this.   

I should add that you now this masseuse- she has been in the UAE for years and you know she has tested negative.  It is unlikely that she even has HIV.  

I see no reason for concern and no need for testing.  I would not waste my money on a PCR test.

Many thanks Doctor for the reassurance.

So to confirm my understanding: 
  • no risk, no need for testing and I am not putting my partner at any risk
  • possible exposure to blood/fluids simply not relevant in the absence of any penetrative (oral/vaginal/anal) sexual activity.

I came across some other information online from what seemed legitimate sources that also estimated the risk even in the case of actual vaginal intercourse at 1-in-1000 or less per instance (is this correct?); which also reassured me as in my mind it was far far more easily transmissible than that which perhaps was adding to my panic.

I appreciate your recommendation that testing is not warranted; however, in the event I want to do a PCR test purely for peace-of-mind, is there any accuracy at 7-10 days? Or would it need to be done later? Again, I realise this is for emotional versus medical reasons.

Your summary is correct.  No penetration, virtually no risk from surface contact with an infected partner’s genital secretions or blood.

The source you mention is also correct.  On average, the risk of infection from a single episode of vaginal intercourse with an HIV infected partner is less than 1 in 1000.  Actually, more recent estimates would put the risk at about 1 in2500.

I do understand the comfort that a negative test might provide.  A negative HIV RNA PCR test at any time beyond 11 days in virtually conclusive.  Testing earlier is not and just how much less is hard to say- there are simply not day by day estimates.

Many thanks Doctor. Will see how I feel and proceed with PCR if still feeling anxious at 11 days.
Sorry, and just to confirm, does 'virtually' in this instance means theoretically possible but not a practical consideration / risk?

Final response:

In science there are no absolutes and new discoveries are made all the time.  Perhaps someone sometimes acquired HIV in this way, but there are no proven instances.  Try to relax.
