[Question #11745] Risk: broken condom (On pep) HIV

Hi doctor,
Recently, I got into the polyamorous lifestyle and joined an app called fetlife for gangbang meetings. I met this woman at a motel through this app for a gangbang. Her partner just sat there looking at his phone. Apparently, no one else came, so I just had protected vaginal sex and an unprotected blowjob. During the protected vaginal sex, I heard a pop and in a second or two took out my penis from her vagina and the condom was broken. I put on a new condom and continued. I went to a doctor and though he was against it as he believed it to be low risk, I asked for PEP. My 1st dose was around 5 hrs post exposure. Just to put my mind at ease and make sure I didn't have HIV from before, I tested one week after I started taking the medication using oraquick. Not sure if it is effective to detect earlier infections while I am on PEP. I asked her about STDs and she seemed to have shrugged it off. But it felt as if she was secretive. Since then, I noticed that she is now organizing a bareback gangbang. This got me scared. She follows unsafe practices. She might have hiv. I messaged her from a different account to test her and for bareback she is charging money. So I think she might have been a SW. She doesn't seem to follow safe sex practices. Her behavior made me worry and I am posting this.
1. How much of a risk for HIV am I on? Can I expect to be HIV negative?
2. Since I am on PEP(2 pills: Tivicay; Emtricitabane and tenofovir disoproxil fumarate tablets) and if I complete my 28 days, will I be safe?
3. How long after PEP should I wait for an HIV test? I think Planned parenthood gives third gen tests in Los Angeles.
4. My doctor prescribed the PEP pills for 28 days, but the pharmacy gave me 30 days worth of pills. Should I take it for 30 days instead?
5 I take the pills at the same time every day. However 1 day, I took it 15 mins later. Is that fine?

PS: Correction, my first dose was approximately 4 hrs post exposure.

Welcome to the Forum. Thanks for your questions. Sorry to hear of your condom mishap. I'll address your HIV-related questions below but would suggest that at some point you test for STIs as well, particularly gonorrhea and chlamydia. Your risk for this infections is far higher than your risk for HIV.
1. How much of a risk for HIV am I on? Can I expect to be HIV negative?
If your partner had untreated HIV (effective treatment eliminates risk of HIV transmission), your statistical risk for acquistion of HIV is less than 1 infection per 2000 encounters on average. PEP makes that risk still low. In other words there is more than 99.9% chance you would not be infected, IF she has HIV
2. Since I am on PEP(2 pills: Tivicay; Emtricitabane and tenofovir disoproxil fumarate tablets) and if I complete my 28 days, will I be safe?
PEP of this sort reduces risk for HIV more than 90% on top of the already relatively low risk for infection
3. How long after PEP should I wait for an HIV test? I think Planned parenthood gives third gen tests in Los Angeles.
I suspect PPH uses 4th generation tests- it is the industry standard. Tests at least 30 days after completion of PEP will be conclusive.

Yes, take the 30 day course.
5 I take the pills at the same time every day. However 1 day, I took it 15 mins later. Is that fine?
Minor variation in when you take your pills will not reduce the efficacy of your PEP.

Thank you doctor. That puts my mind more at ease. For what it is worth, I am uncircumcised. Does that reduce the 99.9% number that you gave? And rest assured, I plan to do a full panel test 30 days after my PEP regimen is finished.

That you are uncircumcised does not change my assessment.
PEP is a personal choice based on perceived risk. I agree with you that the partner you describe does not sound as though she practices good sexual health. EWH

Thank you doctor. I won't worry about it too much. I will continue my PEP regimen and get tested later. Wouldn't have been with her if I knew she was like this. She is advocating strong preference for barebacking if you have a negative test. Anyway, it does not matter. I can only test and confirm I don't have anything.

Hi doctor. I know you said you are closing the thread. But I had one final question. I saw online sources saying risk is 1/500 instead of 1/2000. Is that number for something else?Overall do you think I am very safe for HIV? Also, is there any risk for hepatitis?

Also I heard that pep should be best be taken within 2 hrs of exposure. Is it alright I took it around the 4 hr mark?

The internet is not your friend. Scientists agree with the less than 1 in 2000 estimate for genital sex. Don’t be misled by the internet.
Your risk for HIV is low.
The sooner PEP is started the better but standard recommdations are to start it with 72 hours at most.
