[Question #11821] HIV Dr Handsfield Please Cunnilingus exposure

Hello doc, I was scrolling through reddit and read few stories of Men who have contracted HIV through Cunnilingus, It seems to be more common then you guys make it seem. I know the risk is 1/10000 but what would make it riskier ? I didnt notice any sores/ Cutes in my mouth i might of had inflammed/ Swollen Taste bud would that make my chances higher ? Lets say the sex worker had a acute infection. I took at RNA test on the 11th day now im waiting on results.
Also what are the risks i got infected with oral HSV-2 Its been 12 days no symptoms but read on here that most infections have no symptoms.

Client's are not permitted to request which of us responds to questions. Hence I will be replying. FYI, Dr. Handsfield and I have worked together for over 40 years, have published hundreds of papers together and have responded to over 10,000 Forum questions and have NEVER disagree on the facts of our assessments despite difference in our verbiage. Do you really think that my/our replies would change based on hearsay, non-scientifically reviewed internet statements made on Reddit? If you do, I have some swampland in Florida I'd like to sell you. Get real!!!
Your RNA test will be negative, confirming, once again that you did not acquire IV from the virtually no risk encounter you describe.
Your risk of HSV is likewise miniscule. If you were going to develop HSV lesions, they would have appeared within 10 days of the exposure which seems to haunt you.
This assessment is not going to change. If you prefer to put your faith in Reddit, feel free. EWH

Would my HIV RNA at the 11th day result be conclusive or would i need another test at 4 or 6 weeks ? How many people have you seen or heard acquier HIV through Cunnilingus in the years of practice and how many people have tested negative with a RNA at 11 days who later tested postive ?
My oral swab and urine sample for chlamydia and gonorrhea was done at day 4 and another one on day 5 of exposure could it have been to early to detect ? Every medical site on the web states that it can take up to 14 days for conclusive results can you explain to me what the right time is for conclusive results?

HIV RNA PCR test results at any time 11 or more days following an exposure are virtually conclusive. Some experts suggest a repeat test using a 4th generation HIV antigen/antibody test at 6 weeks but we have never seen or heard of a person with a negative HIV RNA PCR test at 11 or more days go on to develop a positive test. In your position, personally I would not bother further testing.

Okay may i ask why on some of your replies on this forum state that RNA is not conclusive until 12 days after but now your telling me 11 days is Conclusive ? I took the RNA test on day 11 of exposure not 12. Do i need to retest ?
Also if i have already tested for gonorrhea and chlamydia oraly what other test do i need to take to make sure i pass nothing to my regular partner from my cunnilingus with a sex worker exposure? I want to make sure i am 100% clear of every possible STI/STD before infecting my regular partner.

I took Penicillin at 6 days after exposure just in case for syphillis, Could i have been infectious within those 6 days ?

HIV RNA PCR tests provide virtually conclusive results at 11 days or beyond. See my response regarding additional testing at 6 weeks.
See my reply regarding your Gonorrhea and chlamydia testing above. No other testing is needed
You were not infectious between your encounter of concern and your unnecessary penicillin shot.
