[Question #11854] Unprotected oral

Good evening doctors and thank you for all you do. I received unprotected oral on 8/19. That night I noticed a dull pain from inside my penis pretty much through the whole shaft up to the head. It comes and goes and still has been for the last 4 days. No burning while urination or discharge. The individual told me they were DDF so I believed them but since having this dull pain has caused some alarm. If there was any discharge from say gonorrhea would that have occurred by now? Also how much would it discharge.

First, oral sex is safe sex: not entirely free of risk, but low chance of any STD and zero risk for some, including HIV. Second, I've never heard of DDF -- probably because I'm of a different generation -- but google suggests you mean "drug and disease free", right? Most people are pretty accurate in judging these things, so it suggests your partner is unlikely to have had any transmissible STD. Third, your symptoms don't concern me at all. There are no infections -- STDs or anything else -- that can start to cause symptoms within several hours. 24-36 hours is the minimum. And no STD or infection symptoms are likely to come and go. Both the nature of the pain and the fact that it comes and goes strongly suggest it has an emotional origin -- i.e. anxiety magnifying trivial symptoms or normal body sensations that otherwise would be ignored or not even noticed. In any case, there is no STD known to cause symptoms like this.
How much discharge? It can be quite small -- but if not obviously visible, and if no staining of your underwear, then you don't have any discharge.
I really don't see cause for worry at this point. My advice is to sit tight for another 3-4 days and see what happens. I would expect the discomfort to fade once you become confident that nothing is seriously wrong. And also to see if other symptoms (discharge, painful urination, penile sores) develop -- in which case you'll need to get medical care in person. However, if the discomfort stops or no other symptoms appear, I would advise no need for testing.

Thanks you for the response sir. Yes that is correct on the DDF. today is day 5 still have that weird feeling as well as the tip feels more sensitive then usual and notice it as I’m walking and rubbing my clothing. Still no pain or discharge and will monitor for a few more days and will respond then with an update.

Hello again sir just an update. There still isn’t any discharge at all. No stained underwear. No burning while urination but the feeling is still in my urethra. Not there all the time as I try not to think about it but I notice if I lay on my stomach and pressure is on it then it starts to sting. The pain is sometimes dull and other times a sting in the head of the penis. I have been urinating a lot over the last few days but still no pain there. I know it’s not a normal sensation bc it does have a sting to it but not sure if it’s my mind causing the pain. Also no blisters or anything. Do you think this is something I need to be worried about? When can I consider having sex again? Also if I was to get tested and it was positive how soon after start of treatment can you have sex? Can the urethra just be inflamed for unknown reasons and it just needs time to heal? I’m trying not to go down the rabbit hole online like so many people do but every search I do do pain in urethra it pops up with sti and burning and discharge. Again thank you for all you do in helping others

Forgot to add that I have noticed with the frequent urination a lot at night but after I’m done I start to dribble urine for a few seconds before it stops. This is new for me and causes some urine to drip into my underwear. Can this be more a prostate issue I’m dealing with rather sti and the timing of it was just perfect for the unprotected oral? Hello sir, I have one more question for you and I hope that is OK. I just sent in an additional question but for some reason it went under somebody else’s topic. The last question I have is if it is an STI and I have some Doxy left on hand if I would’ve take the Doxy, even though I know that is not the first line of the fence against gonorrhea would my symptoms ultimately get better With the urethra or no because that is not the appropriate anabiotic for gonorrhea if that’s what it was

The symptoms you describe are not caused by any STD. As noted above, they still sound like probable genitally focused anxiety. I doubt your urethra is "inflamed". By far the nearly exclusive symptom of urethritis is discharge; usually accompanied by painful or "burning" urination, but burning or painful urination alone do NOT indicate STDs. (As it happens, my forum colleague Dr. Hook was the senior investigator on one of the main research studies with this conclusion.) Urinary dribbling also goes along with bladder or urethral muscle spasm, a common component of genitally focused anxiety.
I would say you can safely have sex with your partner now. However, I would understand if you choose to have a urine gonorrhea/chlamydia test first. You definitely can expect negative results. Do not take doxycycline -- certainly not before being tested. It would screw up interpretation of the result. Finally, regardless of the test result, if your symptoms persist it would make sense to see a urologist. I am confident s/he would confirm you have no STD.
