Doc, I had a "risky" PIV protected intercourse with a person of a unknown status that would be considered high risk in Canada. HIV-2 is conclusive at 8 weeks, is that correct? I know that HIV-2 is rare in North America (I am from the United States) : HIV 1+2 AB+HIV1P24 AG, CLA (4th gen blood lab) at 18 days, 23 days, 28 days, 36 days, 49 days, 60 days (~8.5 weeks) ALL came back negative / non reactive, HIV-1 RNA count, SerPlas, Qn, probe (PCR blood lab) at 25 days, negative / non reactive.

It would be helpful to know more about your partner; "considered high risk" isn't very meaningful. But since it was a female partner, probably your risk for HIV was near zero, especially with a condom. My understanding about Canada HIV data is that even among the most sexually active women, including sex workers, well under 1% have HIV; and in most population groups it's on the order of one chance in a thousand. When a woman has untreated HIV, the average risk for her male partner is under one chance in a thousand, and a properly used condom reduces that risk buy ~99%. Mathematically, these figures put your HIV risk at something like 0.01 x 0.001 x 0.01 = 0.0000001, or one chance in 10 million. Perhaps your risk is higher, depending on more information about your partner.

You were way over tested. A single AgAb (4th gen) test at 6+ weeks would have been conclusive, includin; even the 23-28 day results were 98-99% conclusive. And I can't imagine any knowledgeable provider or lab doing all the other tests you had! As for HIV2, indeed it is vanishingly rare in Canada (and the US) -- but at least the 60 day AgAb result was conclusive for HIV2. (And are you sure the PCR test didn't include HIV2? The newer PCR tests detect both viruses.)

For sure you do not have HIV. Don't have any more tests!

Thanks - fan of your work. It was a high end female sw. Despite protection used, a lesson learned to not engage in risky partners

I agree, I was overly tested - its the hypochandriac and ocdness in me. The PCR test name was: HIV-1 RNA Detection and Quantification, Plasma so it was indeed tailored to HIV-1. Practically I am in the clear and no further testing is needed ?

I understood PIV.

In general, it is believed that escorts -- i.e. "high end" female SWs -- have very low rates of HIV infection. They know the score, use condoms, get tested frequently, and most of their clients are low risk themselves (men like you).

Definitely you "in the clear and no further testing is needed".
