[Question #11882] My test was positive

Hi Terri,

So I dont know if you can see my previous posts and remember but...

The last doctor was correct, my test just came back positive for HSV2. But I do have some questions how I can proceed forward.

So about 1 month ago, I was exposed to HSV2 orally. He had some scabs on his penis, I told him to go to the hospital, and it was in fact HSV2. And yes, I did not see the scabs until after giving him oral sex. We did do mutual masturbation too and his penis did touch my buttocks, but there was no genital to genital touch. So I contacted you and I did a baseline IGG to see if I did have HSV2 antibodies, I did not have it. The plan was to do a new IGG test after 8 weeks had fully passed. However, in between of this wait, I have had other unprotected encounters. Oral sex has been involved in some of them too. 

My main worry is, do you think I may have catched this orally too? My main symptoms genitally right now is shooting pains down my foot, then sores etc on my vulva. However when I went to bed last night, I felt same strong tingling feeling on a spot on my upper lip.

I guess there is no way to be 100 % sure if I have it orally too and I heard it´s rare. Lets say I have it orally too, or not 100 % sure, do you think it is nesesscary to disclose this prior to kissing? I feel pretty comfortable with disclosing that I have it genitally, but I feel honestly pretty sad if I cant never have a spontaneous kiss in my life again.

What do you think?

The shooting tingling pain comes and goes on my lip. It is not comfortable and reminds me a little bit of the shooting pain I have in my foot.

I would say that unless you have something visible to swab test on your lip, there is no way to know for sure if you have it there and because if it is there, it so rarely recurs or sheds, at this point, you don't need to disclose.  If you get any type of skin change on your lip edge or nose, have it swab tested right away.  
So since you have new infection genitally, the people who you have had unprotected intercourse with were at high risk of acquiring HSV 2.  You should make them aware of your HSV 2 status now.

Yes I will tell them. Imagine if I did not insist the first doctor to take a swab, that is mind blowing to me. He was so sure it was bacterial and I had to argue to make him swab my lesion...

Also I was wondering,
1. Is it true that the first outbreak is the worst? Because this is my first outbreak and luckily it is so mild, I am worried that it will get worse. 
2. Do you recommend me taking antivirals if I am not in current relatonship right now and my symptoms are so mild? Or could I actually benefit from taking medicine right now? 
3. I am a bit worried of transmitting this to other people, of course. As long as I dont have sex with them, should I worry about it in daily life? Like towels? What if someone dries their hands after using a towel I used it on my body during outbreak?

The first outbreak is often the worst but some people are totally asymptomatic when they acquire HSV 2.
If you want to reduce transmission to sex partners, it is best to take this daily, 500 mg valacyclovir is the most usual dose
You don't need to worry about transmission in daily life, no, though we do encourage people not to share towels

The reason I ask is because I realized when I have friends over, sometimes I have seen them drying their hands quickly on same towel I use for my body instead of the hand towel. I dont really know why, but would they get herpes if they only used it to dry their hands/face? Or would it be a risk only when using the towel for lower body? Should I not be worried as long as I am not having an outbreak?

The risk of towel transmission is very low.  I would figure out a way to keep your towel separate.
