[Question #11910] Non sexual Hiv transmission

Dear doctor , 
I am male 39 ( heterosexual) I have anxiety disorder ........I have few questions on sexual and non sexual exposures hiv transmission.

1. 2nd June ....very little cut from car handle .........no one previously cut from car handle ......if hypothetical someone cut ........time gap in worst case scenario is atleast 10 minutes .....I don't see any blood on handle ........is there any risk of hiv transmission.

2. 28th June .......eating food with my friend ( I think he is hiv positive ) ......he touch a bowl .....I touch same bowl of sauce after 3-4 minutes and then eat my food on table with same fingers ........I checked bowl and my fingers no blood ..........any risk of hiv ?

3. 14th july I tested negative hiv combo test ( exactly on 42nd day after 2nd June car incident) .
So I am free from 2nd June incident but it's 17 days from 28th June .......am I totally free from hiv 

4. My main question I started prep ( truvada) daily on 14th July after test.......on 20th July I was sitting in car and use napkin to touch my gums there was little ( drop) of blood on napkin .......my daughter 9 year old  was sitting next to me .......I am 100 percent sure I throw napkin (paper)outside my car from window ...........then I have seen my daughter use napkin( paper) from box and she was drinking water and spitting on paper napkin.......... I stop her ..........but later on next day I feel may be she use my napkin by mistake and it touch her lips or tounge ............
Is there any risk of hiv for her .......may be I get infected from sharing food and now she got infection from me .

5.Is it possible that I am negative on 14th July and I can transfer hiv to someone on 20th July ( given that I started prep on 14th July just after 1 hour of test). 
Do prep stop person becoming viremic within in 6 days after hiv combo negative test .

6. I am still on daily prep .
Today ( 4th sept) if I get tested again for hiv and it came out negative after 45 days of 20th July can I say that I can't transfer infection 45 days back .

7.i have started prep on 14th July but I didnot do any sex only I have taken massage with fingering vagina and condom protected blowjob on 21st july from massage girl ( 8th day) no exposure after that  so I think my exposure is zero risk so free from hiv as I am on prep too.

Welcome. I'm happy to address these questions, but of course our expertise and this forum emphasize sexual risks and transmission. A basic response is that other than injection drug use with shared equipment, and rarely in medical situations, there simply are almost no nonsexual transmissions of HIV. Directly to your questions:

1,2. Nobody has ever been known to acquire HIV from contact with blood or body fluids in the environment -- whether door handles, doorknobs, kitchen utensils, toilets, shared food, or anything else you might think of.

3. That negative test result confirms you were not infected 42 days earlier.

4. I'm sorry you got onto PEP after this event. It was unnecessary. All it has done is prolong the time until you can have conclusive test results. But you don't really need testing anyway, since there was no chance you were infected with HIV from the event described. You are very seriously overthinking a bunch of scientifically impossible situations.

5. No, not possible. If you don't have HIV (and test negative), you cannot transmit HIV to anyone else.

6. Testing while on PrEP is not completely reliable. However, since you were not at risk for HIV anyway, it doesn't matter. But a negative HIV test does prove you cannot infect anyone, even with the highest risk sexual contact.

7. I don't understand this, except it is clear you were not at risk for HIV.

You understand your anxiety disorder. And intellectually, I'm sure you understand that you have no chance of having or transmitting HIV. If you are not in professional psychiatric or psychological care, I hope you will consider it. Simply hearing the facts and the science -- whether on this forum or from your own health care provider(s) -- is not the answer. But I do hope these replies help a little bit. Best wishes to you.

Thanks for your reply .........deep down I know I am hiv negative as I don't have any exposure . 
I have an appointment today with psychiatrist I will discuss with him my issues.

1. I just want my daughter safe most important for me ..........can I ask one hypothetical question if napkin( tissue paper) touch blood on gums ( drop only) and if she take this napkin and touch her lips with it .............no transfer of hiv because 
a) hiv is non infectious on inanimate object .
b) hiv virus absorbed on tissue paper can't transfer to others because it stuck in tissue paper ..........
(C) If she ingested little blood from paper it can't transfer hiv to her because incident is similar to sharing food and food don't transfer hiv ......... 

2. One more thing doctor my daughter complain pain in throat and has follicular tonsillitis diagnose by her doctor on 5th august after 16 days of napkin incident .....no fever nothing .....which is cleared by antibiotics within 2 days .........I think it is not ars ....am I right.


Sorry for confusion doctor for last question I ask it again ( 7th question)

7. On 14th July I tested hiv negative on combo test and I wanted to do sex with female CSW so  I started prep not pep ( I think you write by mistake ) but  meet only one CSW due to busy schedule on 21st july only ( 8th day of prep) and do only fingering and condom protected blowjob only ...........which I think is safe activity and I am on prep ( truvada) which make it zero risk am I right ..........condom don't break and I don't have cut on finger nothing . I just want to confirm am I free from above exposure ........no exposure after that I am still taking prep ( truvada).

*Sigh* You are just making your own anxieties worse by asking about HIV transmission or your daughter's symptoms when it is impossible you have HIV. 

1 a,b,c)  All these would be zero risk even if you had HIV. Nobody catches HIV by contact with blood or the virus in their environment.

2. Since you cannot have HIV, you cannot have given to your daughter. HIV is not a possible cause of her symptoms.

7. You are correct: no risk of HIV in these circumstances and no possibility you have HIV.

Thanks for your reply 
I have now able to control my anxiety not 100 percent but I am better now ........after taking medications from my psychiatrist ....I have read many replies on forums .....and decide following steps to save myself even from theoretical risks ........ And want to confirm from you .

1. Can't get hiv from touching contaminated surface like phone , door , toilet etc irrespective of timing  but I will put bandaid whenever I have any minor cut on my hand......this is my main issue these days want to overcome this . Can I say that if skin of hand is intact or minor cut covered with bandaid no possibility of hiv even if I touch any bloody surface.

2. One more issue is with eating food with contaminated hand or spoon or fork ......so decide always clean my hands , spoon etc before eating ............I understand from your replies that there is no risk even there is little blood on it .
I am asking it because I have bleeding gums when I brush . Am I correct no risk from eating food in restaurant? 

3. Sorry for asking this stupid question but I want to ask this last question  Do prep ( truvada daily ) also protect from theoretical risk..... ( non sexual risk  touching blood with cut on hands etc )  .....I think it will protect but want to confirm from you . 

4. Hiv can't enter through hands skin as epidermis is thicker on palm and fingers of hand and for hiv entry it need deeper cut like 4mm deeper......is this correct statement ?

All four of these statements are correct:  no risk for each of them. As for PrEP, yes it works -- but of course you should not take it. Because you are not at risk for HIV, the risk of a serious side effects -- including death -- is of course greater than the chance you will ever have HIV.
