So I’ve asked questions before plus I just looked at the answered questions but sometimes it’s reassuring to hear from you personally. Had unprotected oral and protected sex at massage parlor . 20 days ago , been looking everyday for any herpes also been having urethral discomfort , thinking it’s all mental , went to online dr and ordered a urinalysis with culture , because urinalysis came back fine , they said culture not indicated , can I trust this for Ngu , had a full std panel including hsv 2 in feb , all good .So only activity since test is this incident , I been looking down there with a flashlight , found a little red dot under head of penis , part protected by condom and 20 days later , doesn’t look like herpes , doesn’t fit time frame , in spot of condom protected . I’m I over thinking all this ?

Welcome back -- but this seems to be another inflated concern about a no risk situation. I see no reason for you to be concerned about herpes and would advise you stop examining yourself. Herpes would never show up as "a little red dot" of the penis. I'm also unclear about your mention of NGU. Your symptoms don't fit with NGU, assuming you did not have noticeable abnormal discharge from your penis. I will also add that whenever someone suspects their own symptoms have an emotional or psychological origin ("think it's all mental"), usually they are right.
So the answer is yes, I do believe you are "over thinking all this". But let me know if I have misunderstood something, especially about NGU etc. But I see absolutely no reason to be concerned about herpes.

I have irritation after I pee and throughout the day , I see some spotting in underwear in morning but I have that from time to time , is it normal not to do a culture based on urinalysis ? Would you trust it ? As far as herpes , why aren’t you concerned , due to the fact it was protected ? Does herpes usually present under the condom first time and in clusters ?

Thanks for the more detailed information. It seems likely you have nongonococcal urethritis (NGU) from the unprotected oral sex. This fits with underwear "spotting" in the morning and an irritated feeling in the urethra. Herpes almost never causes these symptoms.) The proper medical evaluation is not urinalysis and culture, which does not detect NGU or other STDs. You need in person exam by a doctor or clinic with experience diagnosing and managing STDs, to evaluate for urethral discharge, look for white blood cells in the urethra, and collect proper STD tests, in particular gonorrhea and chlamydia. If NGU is confirmed (abnormal discharge observted, plus increased WBCs), treatment for NGU (doxycycline) would be advised.
Why is herpes unlikely? Because the chance for any one exposure is under one chance in thousands and neither the nature nor timing of your symptoms fits with herpes. There is no point in testing for herpes.
I will be happy to comment after you have been professionally evaluated by an STD knowledgeable physician. A public health clinic or Planned Parenthood would be a good option (if you're in the US); or their equivalents if you are somewhere else. In the meantime, please do not be overly worried. NGU from oral sex is not at all serious, and not harmful to future sex partners. I know you are hyper-concerned (even obsessed) about herpes, but it truly is not a serious consderation given the nature of your exposure and your symptoms. I look forward to hearing from you one more time after you have been properly evaluated.

I went to my primary dr who I’ve been with forever . This is fifth time in two years that Im convinced I have Ngu and he says it’s in my head that the marks in the underwear only show up on dark underwear and it is from erections at night and I don’t have any excess leakage or any odd color discharge just normal clear . He ordered test for gonorrhea and chlamydia which he said will be negative and will for peace of mind will subscribe doxy or z pack , what treatment do you prefer ?

You should trust your doctor's judgment; I agree it is unlikely you have NGU and I see no need for treatment. But if you're going to do it, doxycycline is preferred.
