[Question #11966] Worried about STI

Last 26th of April I have protected oral vaginal with escort.
Some days after the encounter I feel some disconfort in penis (not discharge and not burning while peeing). I was in a private hospital and the gave Ciprofloxacin 250 mg during five days because an UTI (nitrite reactive band). After that I start to feel scared about HIV, STI,...
In went to my family doctor and she ask for Urocultive, Hiv, Siphylis, ... All negative. More tests done 
HIV PCR for both negative at day 25. 
HIV 4th gen lab test negative at 45, 63, 77, 84 and 117 days.
Syphilis TPA negative at day 25, RPR negative 63 and 117. Aantigen HBV negative at day 63 and day 117.
Urine PCR at day 63 
Urethral and throat swan negative.
 I had sex with my more than a month ago (last exposure) and now she has some pain and burn whilie urination. So i feel scared about it.

1. Could levofloxacin  afected the first urocultive.
2. The PCR order two not peeing at least two hours before. I did it maybe between one hour and one and a half. Could be a false negative due to the time?
3. The urethral exudate should detect gonorhea, clamidi? Just found Gram Positive Cocci. Result was normal saphrofital. 
4. HIV and other STI am I free with thta test.

I'm afraid you went down an unnecessary rabbit hole. No STD causes "discomfort in the penis" as the only symptom, and with protected oral sex you were at zero risk for all the conditions for which you were tested -- and most escorts (expensive female sex workers by appointment) have no STDs anyway. All your test results were unnecessary, but the negative results are reliable. You also were treated with antibiotics that you didn't need. Both you and your doctor overreacted to this situation. To your specific questions:

1. I'm not familiar with "urocultive" but the name implies a test for urinary tract infection. It detects no STD. It could have made the test negative by curing a UTI before you were tested, but UTI doesn't result from sex anyway.
2. Presumably the PCRs were for gonorrhea and chlamydia, yes? Even though it is often advised not to urinate before testing, it actually makes no difference -- and you could not have had these infections anyway.
3. Yes, this was a normal test result indicating no STD.
4. The negative HIV and syphilis tests are conclusive. You were WAY over tested and over treated! (And the antibiotics you had were not adequate for STDs anyway, except for chlamydia.)

Sorry doctor I know that maybe I have been overtested. This was the first time in my life I have sex with a escort. I am married and also stupid. The problem was going to the hospital and thet gave the antibiotic, after that I had white tongue and is when I start to think to much. My urologist examinate me and saud exactly the same.I am from Spain, Urocultive is Urine Culture. Thats what I tried to explain. That was made Two days after Ciprofloxacin. 

1. Why do you think I have that little pain in my penis? The head was very red. Do you think that was due to cleaning to much or examinate every day looking for some chancre or herpes? 
2. I also asked some doctor and told me exactly what you said. But I am still wortied about my wife. This is the first time in her life with something similar to an UTI. I have sex with her after the test. Maybe is a little chance the test gave me a false negative?  Should I test a Urine PCR again?

I would like to thank you and Dr. Hook for all that you have done  and if its helpful for other people I just want to sat it,s better to speak with and STI specialist because they are going to guide us  properly and don,t take medicins without a proper diagnosis. I have some online chats with doctors that continue ordering test aftwr six months.

There's no "maybe" about over testing!

White tongue is meaningless as a health indicator; it happens all the time in entirely healthy people or with minor colds etc.

1. I cannot say the cause, but one possibility is anxiety (over a sexual decision you are concerned about), magnifying trivial symptoms or normal body sensations that you otherwise would disregard or maybe not even notice. For certain it was not due to any infection.
2. Having had the same advice from your doctor (apparently two of them?) and on this forum, you need to stop worrying about your wife. She is not at risk for any infection of any kind.

Just to clarify, the sex was vaginal and oral. I ejaculate in the condom. I don,t know if this change your recomendations.

After the antibiotic I have some sore throat (not heavy, not fever), with white mount and tonsil. The doctor gave me Mycostatin and told me that it,s normal after taking antibiotics. Is that correct? Nevertheles in the throat swat not Candida was found. 

Sorry just wanted to be sure about this and the situation that was also protected vaginal sex not only oral sex

Una ves mas, esta información no cambia nada. The nature of the exposure needed no clarification, and the white mouth and tonsils were a side effect of the antibiotic treatment you did not need.

Do your best to move on with no further worry about these events. Please no more testing or treatment, and resume normal sexual relations at home.
