[Question #12004] Protected sex with asian massage worker

Hello, I have probably a pretty typical case but for the sake of my anxiety I thought I’d ask anyways. 

Two days ago I went to an asian massage parlor and had brief protected intercourse with the worker. I made sure the condom was applied properly, covering my penis completely and leaving space at the top for the reservoir. The entire encounter only lasted maybe five minutes, and as far as I know there was no direct contact with the worker’s genitals whatsoever, but obviously I could be wrong about that. The condom was completely intact afterwards and only came off once I had finished. I drank plenty of water afterwards and urinated frequently. I am currently experiencing some mild irritation with my penis, but there is no pain and no visual changes. 

What, if any, STIs am I at risk for, and do you feel that I need to be tested?

Welcome to our Forum.  Thanks for your questions.  I'll be glad to comment.

As a generalization, most of our clients over estimate their risk for acquistion of STIs as a result of encounters with commercial sex workers (CSWs).  While there certainly is a spectrum of CSWs, most who work in brothels, massage parlors or through on-line services are relatively low risk partners.  They are aware of their risks and take steps to avoid them including frequent check ups.  After all, giving a client an STI would be bad for business.  Consequently, most CSEs do not have STIs and even if and when they do, most single encounters do not result in infection.  In your case, the risk of infection is lower still since when condoms are worn throughout an encounter and do not break (condoms break about 1% of the time they are used, more often with new partners and with anal intercourse) they virtually eliminate any risk for HIV, gonorrhea, chlamydia and trichomonas.  They also reduce the risk for acquistion of herpes and syphilis more than 50%.

I would characterize the encounter you describe as low risk and in the absence of symptoms (I would not count the irritation you describe unless it persists or worsens), I probably would not both to test at all.

Hello again,

So the irritation in my penis has stayed, and is slightly worse now. There are still no visual changes to my penis. It doesn’t hurt to urinate, but there is a constant slight burning sensation now. Could this be herpes in your opinion?

Most typically herpes would be accompanied by visible lesions on the outside of the penis.  The irritation you are experiencing could be non-gonococcal urethritis (NGU) although your condom should have prevented this.  A urinalysis performed on a urine specimen collected as you begin to urinate might be helpful.  If moderate or more white blood cells are present, there could be a urinary tract infection or some other sort of irritation.  If there are no or few WBCs, perhaps you are looking too hard for symptoms and noting what are normal sensations that are typically overlooked.  Given your condom use, this should not be an STI.  EWH---
