[Question #12067] HIV exposure?

Hi I had sex with an escort just now.. during intercourse after I pulled out to switch positions.. the condom slipped off and was in her vagina.. don’t know how long I was exposed. And I have hyperpigmentation marks from penile biopsy almost a month ago.. I don’t think there was any openings in skin but idk.. also my penis was wet when condom slipped off.. not sure if my pre cum, her vaginal fluid, or the lube that was used..or all also after inspection I thought the condom had a small tear in it after but I squeezed the end hard like trying to pop a balloon and no fluid came out the other side.. we changed condoms and continued.. don’t know her status.. she’s 21 Colombian.. doesn’t speak English.. she didn’t seem overly concerned.. should I get pep etc? Nervous here

Welcome back yet again.
We do not give specific advice either for or against PEP to prevent HIV. The decision should be made with a health care provider or clinic who understands the local frequency of HIV, treatment of HIV in such partners, and so on. I would make some general observations, however. First, "escort" usually implies an expensive female sex worker by appointment. Such persons tend to care for their health, use condoms consistently, get tested frequently, and have partners mostly at low risk for HIV themselves (men like you) -- and therefore are unlikely to have HIV. Second, with unprotected vaginal sex the estimated transmission rare of HIV to a male partner is well under one chance in a thousand. If we assume one chance in a thousand your partner has HIV, your HIV risk calculates to be under one in a million. Most people probably would consider that too low a risk to seek PEP; even testing for HIV is optional. However, it's up to you if you would like to see a local doctor or clinic to advise you.
The other info you provide -- penile hyperpigmentation, the condom "water test", or the lubricant used make no difference one way or the other. Except to point out that we always recommend against water testing of ocnodms. It's based on the urban myth of microscopic leaks that permit virus transmission. If a condom doesn't break wide open, protection is complete.
I hope this reply is helpful. Let me know if anything isn't clear.

Should I even test, or is it too negligible? If I do test what test can show results with the quickest incubation period? Should I worry about having sex with future Partners if I don’t test. Thx

Testing is your decision, not mine. From a medical/risk standpoint, I do not consider it necessary. However, you'll have to decide yourself whether you would feel better about the situation if you have tested negative; it's up to you. You could have a conclusive HIV RNA PCR ("viral load") test 11 days after the exposure; or an antigen-antibody (AgAb, "4th generation") blood test at 6 weeks. Most people in your situation would avoid sex with others until they know for certain, but this also is up to you.---

Thank you.. just really worried about this situation.. but based on how your responses are tailored.. risk is very low for this incident but not impossible.. so low it’s not worth stressing over correct? and there’s a strong chance HIV was not transmitted?.. again I don’t know her status

"risk is very low for this incident but not impossible...." Good summary, I agree. Of course you don't know her status -- but among escorts like your partner, probably under one in a thousand has HIV, so you can safely assume she doesn't have it. If somehow I were in your situation and had similar exposure once a month, I would have an HIV test once a year and definitely not after each event.

Looking back over the years, this is your twelfth forum question and your second or maybe third about very similar exposures and HIV testing. (Twelve isn't a forum record, but it's not far off!) Our advice has been pretty much the same each time. It shouldn't be necessary to ask about similar events again; you will know our advice ahead of time. So let's make this the last about such events. OK?
That completes this thread. I hope the discussion and the others have been helpful. Best wishes and stay safe.
