Hello again.. I have an additional question from my last question.. I got the hint last time about repeated questions on the same category.. and it is accurate.. I’m just somewhat going through somewhat of a mental breakdown right now..also this is the only forum I trust, based off credentials and responses.. I know you’re not a psychiatrist and don’t have to deal with this nonsense.. but I have no where else to go for accurate guidance. I go to Costa Rica on 10/15 for a Bachelor party.. only reason why I booked an escort was to get used to sex again cause of the trip coming.. there will be sex there.. I will be protected of course but not thinking this way is foolish.. then I’m changing my lifestyle… my question is based off my recent post: condom slip from escort, if you were my dr would you recommend pep or hiv Rna test.?. I spoke to std clinic they told me 9-11 days after exposure is 95% accurate.. I know based on last response you can’t make recommendations from pep.. not sure cause of e&o reasons or not etc.. but last year a reccomendation not to use pep was sent.. just looking for a response, based if you was my doctor. And again this is my last post on the forum here.. i need and will be changing my lifestyle.. hope it’s not too late. I can’t go through this anymore mentally.. thank you for dealing with me over the years.. I feel like I should be sending holiday cards at this point

Welcome back to the Forum although I'm sorry you felt the need. I read your recent interaction with Dr. Handsfield and agree with all that he said, as well as his suggestion that you really are clearly coming back again with similar questions about low risk exposures. I agree that you to figure how to address your insecurities, whether that be through counseling, continued regular preventive measures (i.e. condom use in particular), or both.
Dr. Handsfield has already explained to you the reasons that any single sex act, and particularly a single protected sex act with a commercial sex worker is very, very low. As Dr. Handfield has already told you, given your recent condom mishap, testing is a personal decision and is likely to be negative. We typically do not recommend testing following every single exposure to a new partner, particularly if condoms are used. If you came to my clinic following your recent exposure, I would not favor prescribing PEP, just as I would not suggest anything other than safe, condom protected preventative measures for your forthcoming trip to Costa Rica. Thank you and it can be assumed that it is likely 99.9% chance im not positive based on this exposure.. read that on another one of your posts with a similar occurrence.. also you don’t see a problem with protected intercourse going forward if I don’t test before? Also what is the likelihood of a false positive from hiv rna test. Thanks

As I've already said, the exposure you describe was low risk and we feel it is not necessary to test following every low or very low risk exposure such as the one you have described. As Dr. Handsfield has already pointed out, testing however is always a personal decision.
False positive tests are rarely seen with current HIV RNA PCR tests. There are not specific figures other than the statement that HIV RNA tests are 99.9% accurate. EWH

Thank you and the Chance I describe is 1-2400 exposures and that’s assuming she has it.. what does that equate too 99.6% chance of no transmission.. that sounds very low.. which eases my concerns

I agree with your estimate.
