[Question #12081] Post-op Trans Female STD Risk

Hello Doctors:
I recently began dating a post-op trans female.  She underwent reassignment surgery including vaginoplasty more than 20 years ago.  I am heterosexual and plan to treat this as I would with any new relationship and discuss STD testing at the appropriate time.  I know that HIV rates are higher among transgender females than cisgender females presumably because of the prevalence of anal sex among the former group.  I'm curious however whether the risk of STD acquisition is any different for a post-op trans female.  Is it comparable to cisgender women?  My partner is a stable professional in the medical field if that makes any difference in the equation.  I guess I'm just a little curious from a biological standpoint if there is any difference in the risk of STD transmission for someone who has undergone vaginoplasty.

Welcome to the Forum.  Thanks for your questions.  I'll do my best to help although this is an area in which there are few high quality studies to inform this response.

As an individual, your new partner's risk is not specifically relate to her status as a trans-sexual person but is related to her past sexual activities, risk taking and history.  As a group trans-sexual person tend to have higher rates of HIV and other STIs than cis-gender persons and have STI rates similar or, if anything lower than men who have sex with other men.  Her risk of HIV relates to her prior sexual activities which might have put her at risk for HIV.  Her present risk relate to her current and continuing risk for STIs
