[Question #12102] Scared for gonorrhea and Chlamydia after receiving oral sex

Thank you doctor for taking my question. About 10 days ago, I had a one time unprotected oral sex episode with a Romanian sex worker at a brothel in Germany. After 2 to 3 days I started a little tingling when I started to pee. And next 1 or 2 days I had little itchy feeling when I pee. I got scared of gonorrhea and Chlamydia. I have no burning sensation or no discharge at all. I already had sex with my wife and now I am worried that if I cought something I must have given it to my wife. Can you please help me understand my risk and what to do. Thank you for your service.

Welcome to our Forum. Thanks for your questions. I'll be glad to comment.
The encounter you describe was a virtually no risk event. In Germany, by law, commercial sex workers are checked regularly for STIs, making it unlikely that your partner had any STI. Further, among penetrative sexual encounters, receipt of oral sex is amongst the lowest. There are no proven cases of HIV acquired from receipt of oral sex and IF your partner had an oral STI (unlikely) there is little chance that her infection was transmitted to you. The major STI transmitted by receipt of oral sex is gonorrhea and over 90% of men with gonorrhea have an obvious penile discharge with 3-5 days of exposure. On the other hand, penile itching is not a sign of any STI although it is the sort of symptom that we see when worried persons are looking for symptoms. In such situations, persons without infection tend to notice normal sensations as itching.
In your situation I would have little concern. If you are worried, at this time a urine specimen tested for gonorrhea will provide accurate results. Many of our clients appreciate having a negative test result. If you choose to test, I anticipate that your test will show that you are not infected.

Thank you sir for the quick response. I have one more question. What about Chlamydia or NGU . Are they easily transmitted via oral sex?

Sorry doctor but I forgot to add new question. 4 days after my encounter I took 1 g of azithromycin on my own . I know it was a mistake to treat myself without discussing with a doctor. My concern is that 24 hours after the azithromycin I had sex with my wife. Now I am worried that if I go and have the test done for Chlamydia and gonorrhea, it will be negative for Chlamydia and there would be a confusion that what if I got Chlamydia and gave it to my wife before medicine cleared it from my body. So can you please tell me if that's a possibility?

Chlamydia following receipt of oral sex is very unusual so, in addition to the information I have already provided about gonorrhea, the same is true for chlamydia/NGU.
Your self-medication with azithromycin further reduces the already virtually no risk nature of your exposure.

Last question doctor. How soon after taking 1g azithromycin I am not able to give anything to my wife just in case I got something. I had sex with my wife 24 hours after taking azithromycin. Also that was day 5 post exposure so I wouldn't even have any symptoms of ngu or Chlamydia at that point. I am scared that I probably have given something to my wife. I am regretting my decision so much.

There is no evidence that you ever needed the azithromycin and having done so just adds to the confusion. Azithromycin enters the blood stream within an hour or two of having taken it and would reduce the likelyhood of transmitting any infection present that was susceptible to azithromycin soon after that.
There is no evidence that you have given your wife anything
