[Question #12105] Stds/herpes type symptoms

Good afternoon doctors I have a question involving an exposure I had  about 3 months ago. I had protected anal sex with a male, I am also a male. The act involved protected anal sex. Unprotected receptive oral, I received but did not give. We did making out, and we both rubbed our penises together, for a few minutes while we made out. At the end I made sure , and the condom was completely in tact, and never broke. The gentleman told me they were on prerp. So I am not too worried about Hiv. Also I recently got checked for all stds, can’t recall if Herpes Igg was involved. I may have to go back to doctors to confirm. But got the full std panel, and all came back negative including Hiv. Whithin a week I have noticed some slight itching and some slight pain. At times It seems like a paper cut on my penis , would be best to describe it. But overall never noticed any visible sores. I am now going on about 3 months , and have noticed a bit of itching around the genitals, and on shaft on my penis, here and there . Have noticed redness also on different occasions that last maybe 2,3 days.  I noticed now maybe for the second time 2 or 3 little non raised white spots by the shaft. They seem to never be there . Not do I know exactly what it is. It looks unraise and not like Blisters. The timing seems to mark herpes especially with all the negative std tests. I have also seen some whiteness on penis. Not sure if it’s considered discharge or it crusting. I received head once by my gf about 2
Months after exposure. And it hurt really bad. But since then Have not had that type of harsh pain. 1)What do you think this could possibly be 2) how risky was my exposure 3) just any feedback you may have on this thank you

Hey Doc I completely forgot one important part, during the encounter there was a point where we did it for a bit ,penetrarion with a condomand she weirdly got up out of nowhere, and said she had to douch(clean her butt/hole) again, not sure if I am spelling it correct, and so I took off the condom. And threw it on the floor. I don’t believe I washed my hands after possible touching some light stool, and while I waited for her to do that, I jacked off a bit, may have been with the same hand. And when I put on a new condom, I always put a finger through to see if it is the correct side, I am wondering if the split two, three seconds of touching the condom, and possible some fluids/ stool, and then jacking off would increase

The contact with the condom is not really a risk for HSV acquisition.  And I don't believe that the initial contact with another male was a risk either.  You were quite careful, though I can't tell if you were bottom or top with the anal sex.  If you were top, and the condom stayed intact, you are likely at no risk for HSV.  And if you were bottom, then you needed to be sure that the partner used a condom throughout the contact.  The white spots you describe do not sound herpetic - you don't say that they were ulcerations or blisters, just white spots, right?

Hey sorry I was the top, so I was the one who put on the condom, and I took it off, and did the water trick, to make sure it didn’t break. Also yes just white spots with a bit of white stuff on the foreskin. I was diagnosed with chalymidia 5 years ago From a girl . So after after that I always use condoms for protective sex. Unless a new gf, and we get checked before we start unprotected. So basically I say that to say I check my penis alot after encounters, ever since then. And I have never seen anything like this. You seem not too worried about the condom part and me neither. My bigger worry is more of the unprotected oral sex. Also I forgot to mention the rubbing of each others genitals together, like they touched for like 3 minutes, and I believe they had precum , we did that without condom use. 1) what would be my chances of getting it from the unprotected oral 2) and how about from the rubbing on penises, together unprotected, with them possibly having some precum on the head of the penis?

The HSV that you would acquire from receiving oral sex is almost always HSV 1.  Have you ever had a cold sore on your lip or in you nose?  If yes, then you already have HSV 1 infection.  The rubbing together is a concern about skin to skin contact, not precum.  But even that seems very low risk to me.  You should have a professional look at the white spots in person for clarity

There has only been once where I have ever seen something that looks like a cold sore, but nor sure if it was that or a pimple. But have not seen that in about maybe 5 years. So I am not sure if I have ever gotten them. And yes my main concern I guess is the genital rubbing,  1) have you ever seen many people say they think they may have aquired it that way, by rubbing penises together, and stroking it? Also I have some good news that has put my mind at 85% ease but still not 100% I went to a dermatologist yesterday, he did not swab anything, but instantly looked at my penis , and said the white stuff looks like candida balanitis. And he said he has so many men that come for that very problem. And prescribed me a cream, that I will receive on Monday, he did A 3 in 1 cream of some sort. My biggest worry Terri is that I started seeing those white things whithin a week of The first male encounter. Which is usually when genital herpes attacks, 2) do you think that makes it more possible to be herpes or shear weird coincidence. Also online it says one of the causes of candida balanitis that isn’t hygenic reasons is having one of three stds which include chlaymidia, ghonno, and genital herpes(two of which I tested negative for) that along with the timing of symptoms is freaking me out maybe it is being causes by genital herpes 3) do you agree or think I am over reacting 4) do you think I should get a herpes igg test, I hear so much contradicting stuff about it, and even had an old Doctor I used to go to , tell me he tells all his patients not to get it, to go the swab route, what is your opinion on that. 5) I forgor to mention to my doctor not only does my shaft itch also around the genitals, a little above it between the belly button and peni, my testicles, and a bit near the anus. Does that sound more like herpes to you or could that also be from the candida?

I honestly don't think that there is much risk from the genital to genital rubbing.  I've not seen a case of HSV being transmitted in this way.  The issue with the IgG test is the high rate of false positive results.  If you did decide to do the IgG test, you should be fully and completely ready to proceed to the best confirmatory test we have now, the herpes western blot from the University of Washington.  It is expensive but can be done from most anywhere in the US.  I think the itching is far more likely candida than HSV, honestly.  and it is very common in men, true.
