2 days ago was with a trans csw. Performed oral on eachother with condoms on.  I tried to give anal as top with condom still on.  erection wasn’t strong enough so it was pushing with minor penetration.  My hands touched csw’s inside thigh/anal area some. Pulled out and manually came with condom still on.  Semen seemed contained.  What is the risk of hiv, gonorrhea, syphilis, chlamydia, and trich, and what testing/guidance would you suggest?

Welcome to our Forum.  Thanks for your questions.  I’ll be glad to comment.  The encounter you describe was close to no risk.  Most people do not have STIs and most single exposures do not lead to infection, even when a partner has an STI. 

 Regarding the encounter you describe, condom protected sex is safe sex and there is no reason to think that your condom failed.  Personally, I would have no concern about STI risk and, in the absence of signs or symptoms I probably would not even bother to test.  This was a low risk encounter.  EWH

1. To attempt to prevent syphilis, gonorrhea and chlamydia I took cefixime (800mg) the day after and started a 7 daycourse of doxycycline (2x 100mg).  Do I need to consider 14days?

2. What is your opinion on whether trich is possible in this situation?

3. Should I avoid sex with my partner for some time?

I have read that syph, gon, & clam can spread via skin to skin and oral. There was some skin to skin w/ the anal/genital areas altho condoms on. Just for info, could I ask some detail on why it isn’t concern for the above STIs?

As you know, we provide up to 3 responses to each client's questions.  This will be my final response.  Following this the thread will be closed and there should be no need for further questions about possible infections this virtually no risk encounter.  If you remain concerns, test at exposed sites.  I am confident that the tests will be negative.

I suspect that you have been on the internet.  That's a bad idea.  Much of what is to be found there is incorrect and misleading.

Transmission of syphilis, which is far less common than gonorrhea and chlamydia can theoretically be transmitted through skin-to skin direct contact with an active syphilis lesion.  Condoms reduce the already low risk of syphilis by more than 60%.   The idea that skin-to-skin contact with genital secretions can transmit gonorrhea and chlamydia is incorrect.  As long as the head of the penis is covered you are virtually completely protected IF your partner happened to be infected.
