[Question #12131] Prevention

|
Everything is clear.....
So I was interested in prep.
They prescribed descovy.
1. Is that effective only as a daily pill or is that also used for on demand?.
2. Should I ask for truvada?
3. I'm possibly interested in having sex with ts/men/woman so will that descovy work for me?
4. How long / how many days will it take to be effective in my body?
I'm only interested in topping not bottoming.
5. Is there anything else I should know?

Welcome to the Forum. Thanks for the questions. I'll be glad to comment. There are several different HIV PrEP regimens recommended. All work well.
1. Is that effective only as a daily pill or is that also used for on demand?.
Descovy works just as well as Truvada and has slightly fewer side effects
2. Should I ask for truvada?
If you are tolerating the Descovy well, I would continue it. It works well.
3. I'm possibly interested in having sex with ts/men/woman so will that descovy work for me?
Absolutely
4. How long / how many days will it take to be effective in my body?
PrEP works immediately after taking it but is not maximally effective until it has been taken for 7 days.
5. Is there anything else I should know?
I think you've covered it.
I hope this information is helpful EWH

1.The first question you might if miss understood it.... there's a daily regimen and there's an off label on demand as needed regimen. I heard truvada was good for on demand but unsure of descovy.
2. Is that 7 days max protection for bottom or top?. Not many studies with people having vaginal sex.
3.I've had protected vaginal sex with prob over 10 sex workers whether they were street, escort, massage etc....
What are the chances that I've came across hpv or hsv but my body/immune system keeps it at bay.
Never had warts or sores

Thanks for the clarification. In reply
1. There are fewer data for PrEP on demand than for daily PrEP. The three main studies that describe PrEP on demand all used Truvada although logically Descovy should be equivalent. No PrEP on demand regimen is currently FDA approved although the data on it's effectiveness are quite strong
2. The risk for infection is greater for persons who are receiving rectal intercourse than for "tops". Hence the longer run-in period is probably more important in that situation
3. As a Sexually active person odds are high that you have already acquired HPV. It's harder to say about HSV. Testing for either of these infections is not recommended however vaccination for HPV is highly recommended for just about all sexually active persons, even those who know they have HPV since the vaccine covers the 9 most common HPV types and it unlikely that you have had all 9 types.
You have 0ne follow-up remaining. EWH
