Hello. I went to urology because of the burning sensation on my penis after suspicious sexual intercourse (unprotected vaginal and oral escort), and a urethral swab was taken. Chlamydia and ureaplasma were positive. I used 2x decefin needle, 14 tablets of tetradox, 14 tablets of ciprasid and 2x20 tablets of minocycline as a result of the antibiogram for ureaplasma. I had a semen test 6 days later and a urethral swab 14 days later and everything came back negative. During this period, the 31st day duo ultra (Hiv, syphilis, hepatitis b and c) 65th day and 88th day repeat (Hiv, syphilis, hepatitis) tests are all negative. I told him that the burning sensation in my penis was not completely gone and doctor told me that it would take some time and that it was not a problem. Indeed, I am 90% better in 2 weeks. It was June 28 when I was diagnosed. We are currently on October 24th. There is someone I care about in my life, I just had unprotected sex with her. I'm afraid of infecting him with something. Could there be any problem with this? Or should I listen to my doctor and move on? Thanks.

My main response is to agree with your doctor that the infection probably is entirely gone and it is very unlikely you have anything that could be transmitted to your main partner or that could harm her. More details are below about my reasoning. But first, some comments about the initial diagnosis and your treatment.

I'm interested to know where you are located. Your only problem was the chlamydia. Most STD experts agree Ureaplasma is harmless and does not need treatment. There are different understandings in some countries, but in the US, there is nearly universal agreement among STD experts that nobody should be tested for it except in unusual circumstances. The only treatment you needed was the doxycycline (Tetradox) -- and 100 mg twice daily for a week (i.e. 14 tablets) is exactly the recommended dose for chlamydia. The ceftriaxone (Defecin) probably was given before your test results were available, in case of gonorrhea. The ciprofloxacin (Ciprasid) was unnecessary; it has no effect against chlamydia and is unreliable for gonorrhea. I don't understand why minocycline was included:  it is nearly identical to doxycycline and anything treated with one is equally treated by the other.

It was good you were tested for the other infections. You can be completely confident you do not have HIV, syphilis, or hepatitis B or C.

So although you were over-treated, and perhaps went through some unnecessary anxiety about the other infections and all those antibiotics, chlamydia never fails to be cured by doxycycline.

What about your continuing penile "burning"? The main symptom of urethritis caused by chlamydia is discharge. (Did you also have abnormal discharge from your penis before being treated?) Mild burning alone usually is not due to infection if no discharge is present. Anxiety often is responsible; the entire experience might have made you more aware of minor symptoms or even normal body sensations that otherwise would not bother you or that you might not even notice. I'm guessing your urologist might have said something like this, right?

I will send 2 separate messages to explain all the details and myself better to you because the site does not accept this, I hope there will be no problem.

I live in Turkey. Yes, decefin tetradox and ciprasid were prescribed before the test results came out. Test results were positive for chlamydia and ureaplasma, and an antibiogram for ureaplasma was studied. And it was only sensitive to minocycline. My doctor told me that I should use this, it was a dangerous infection, and that if I was going to have intercourse during this period, I should use a double condom and if it did not go away, I would be sent to the infection department. All these psychological problems and concerns started after that.

I used Tetradox and ciprasid simultaneously for 7 days. It seemed like my burning had stopped just before the antibiotics ran out and it started again.

As for the discharge, there was never anything like pus. My doctor also checked and said he couldn't see any discharge. I used to have white spots on my underwear from time to time (in the form of small dots). And while I had these, I had a girlfriend of 8 years and we never had vaginal or anal sexual intercourse. Only friction or oral intercourse. She was a virgin. My complaints started after the relationship ended and I was with an escort.

So I cannot say or distinguish that I saw anything visible during this infection. However, after the infection occurred, I constantly checked and milked my penis, so to speak, and nothing was coming out. But I was seeing those white spots.

You said you think the possibility of any contamination is very low. So there's a possibility, right? Meanwhile me. After all these tests, I went to a private laboratory for the last time and had direct microscopy and culture from the semen again. candida, trichomonas etc. nothing reproduced. Only 1 - 2 leukocytes were seen and this was considered normal.

 

By the way, doctor, I had a traumatic masturbation problem for about 15 years. During this period, I did a lot of research and heard that it could cause conditions such as prostatitis. I was blocking the flow of semen.

For the last two weeks I have had almost no burning sensation.

1-Can I have chlamydia or ureaplasma now? (After 2 different tests and those medications)

2-Are my HIV and other tests completely sufficient?

3- Could traumatic masturbation cause a urethral infection that is difficult to get rid of after being infected with these bacteria?

The additional information about ureaplasma doesn't change my evaluation or advice. It had nothing to do with your symptoms and did not need treatment. If it showed susceptibility to minocycline, it probably means that doxycycline wasn't tested. Anyway, it's gone now so nothing more to discuss. I believe there no significant possibility you still are infected with anything that matters. You were over evaluated, over tested, and over treated. With all those antibiotics, a slight possibility for continued urethral irritation might be a yeast infection -- probably not, and if so it probably will clear up on its own. 

1- No, you cannot have chlamydia now. Ureaplasma testing often becomes positive again a few weeks after treatment, but remains harmless. Up to half of all sexually active persons test positve. It is an entirely normal bacteria in the genital trac t.

2- Yes, those tests are sufficient.

3- We are STD experts; I have no opinion or views about "traumatic masturbation" or other non-STD genital problems. That's a question for your urologist, perhaps also with asking about yeast infection.

Can I continue my life normally now? Can we say that there is no risk for the other party if I have unprotected intercourse? I don't want to cause problems for my partner. And if you were me, would you go on without worrying anymore? Thanks.
