[Question #12169] Condom Slip HIV and STI risk

Hello Doctor,
I met a CSW on 28/10/2024 in Toronto. I had protected oral and vaginal sex. However, after ejaculation and pulled out, I noticed that condom was not on my penis. It stuck to her vagina with the opening of the condom is outside the vagina and visible. I took out just by draging it with no effort. I found that the fluid is inside the condom. Also i found a drop of fluid fell on the bed sheet after withdrwal. I want to know
1) Risk of HIV and other STI
2) is it considered as unprotected sex
3) As I am in window period for PEP. Based on my incident Do I need to consider PEP?

Also I know her from quiet some time and she told she gets tested every month or two. She mentioend her last testing was on 21/10/2024. Also she mentioned she got all the doeses of HPV vaccine recently. I think she is very health consious. But I keep worrying because of the condom slip otherwise I would have ignored it considering it as a protected sex.

Welcome back to the forum. Thanks for your continued confidence in our services.

First, congratulations on your attention to sexual safety for your commercial sex events, i.e. consistent condom use. And once again you describe a very safe partner. If your partners classify as escorts -- expensive, working by appointment -- such women generally are believed to have low likelihood of active infection. They are health conscious, test frequently, use condoms, and generally have low risk clients -- men like you.

This exposure therefore was very similar to the previous one you asked about, including condom slippage. As discussed there, it is common for condoms to slip in a condom, and it's a similar event when the penis withdraws, leaving the condom in a partner's vagina. Such events are considered to be completely protected. There is always vaginal fluid contact with penile skin, but that carries little or no risk for any STI.

1) Zero risk or close to it.
2) No, I would consider it well protected sex.
3) I would strongly advise against PEP in this situation. I don't even recommend testing for anything.

I hope these comments are helpful. Let me know if anything isn't clear.

Hello Dr. Handsfield,
Thankyou for your promt response.
Based on your advice I didnot go for testing due to the last incident which I understood it as the protected one. I thought it as the protected one intially, but I was not able to covince myself , so I posted the question here. I had one follow up question. Based on the description I gave Do you think the condom has slipped during the withdrwal only or during the intercourse.

Based on logic, I think it slipped during the withdrwal as the condom opening was outside and fluid was inside. Otherwise , if it had slipped during the intercourse condom should have ended up inside the vagina. Am I correct in this aspect.

Once again, thankyou for the response. It has eased my mind.

I agree with your logic.---
