[Question #12217] Anxious and paranoid of hiv and fear

Dear Dr,

Thanks for your continuous support.
I am returning back with my question .
My exposure was MSM with no anal penetrative sex and no oral sex . Based on last time advise by Drs here , i didn't do any testing.
Assuming two years of after exposure , and at latent hiv stage , can the wbc and rbc normal in peripheral blood test.
Also recently i was diagnosed with hpylori and just completed antibiotics .
Biopsy was done on stomach , colon, esophagus and correlate with h pylori . Can histopathological examination of tissue can detect presence of hiv cell ?

I noticed over the years my weight is reduced 4% . Is it can be due to h pylori infection or latent hiv stage can cause excessive weight loss . I also experience dry mouth . i am vaper and is it can be causing dry mouth or h pylori?
A person who is hiv positive with undetectable viral load for more than year , can cause infection ?
So far i did not experience any ARS from my exposure 2 years ago , but i only involve in frottage , rubbing penises glans together with precum or ejaculate , kissing . I never into oral and anal .
Also what is the likely hood of infection from 1 unprotected anal as receptive partner (just for my knowledge).

Look forward for your reply.

Apparently you have used a different user and and email account for this question so I do not have the benefit of reviewing any previous questions to provide context for this response.  Straight to your questions.

Yes, the RBC and WBC counts can be normal in persons with latent HIV.

No, a biopsy showing H. pylori would not show changes suggestive of HIV.

You have no reason to be worried about HIV- the encounter you describe above is an entirely no risk situation.  It would not normally suggest a need for testing but if you are continuing to worry about the no risk exposure, testing may be a good idea to eliminate any lingering concerns you have about your no risk exposure.  Based on what you say above, I am confident that the test will be negative and that result will be conclusive.

The risk of acquiring HIV from a single episode of receptive rectal intercourse with a person who has untreated HIV is about 1%, i.e. chances are greater than 99% that you would not be infected.

Thanks Dr for your reply .
I would like to clarify further :-
1. I m not referring to rbc n wbc count . I m referring to peripheral blood film under microscope . Will latent hiv still show  normal cell type , look .

2. What could be my weight loss cause . My thyroid is normal . My sugar under control. Recent HPE showed no invasive malignancy in Gastrointestinal tract . USG for liver is fatty liver , pancreas , gall bladder ok .
Can persistent unconjugated hyperbilirubenia cause weight loss . Since 2018 , bilirubin is above 30 to 40 ranges .

3. Can my dry mouth due to vape .
or is there any case of dry mouth during latent hiv stage .

4. Still didn’t answer my previous query on undetectable viral load and transmission risk .

5. Any reported case of asymptomatic hiv acute infection . What is likely hood that asymptomatic of acute hiv infection .
6. Can meth usage relax anal muscle until no feel of penetration . I know meth will make people more alert and vigilant . Should feel by right during penetration right .

Replies:

1.  I understood that.  My answer is unchanged.  An isolated elevated monocyte count is not a sign of HIV
2.  Neither HIV not other STI is not causing your weight loss.  Finding out the cause of your weight loss is a question for your doctor.
3.  No dry mouth is not a typical sign of latent HIV
4.  Persons with HIV have detectable viral loads.  This withundetectable HIV viral load cannot transmit HIV.
5.  No!!!  Nothing you have said suggests any reason for concern about asymptomatic HIV.
6.  No!!!

You are worrying entirely too much about HIV.  No change in my recommendations

You have one follow-up remaining.  EWH

Dear Dr ,
1. Undetectable means ? Can’t transmit to others in sex ? Can a person be undetectable for many years ?

2 in general based on database , is there any asymptomatic acute hiv ?
Many online sources said most acute is asymptomatic.

4. When u said “ No” to previous question no 6 , does that mean under meth influence a person should feel when penetrated their anal ? Or meth will not relax the anal muscle .
Thanks for your concern and time Dr.

This will be the final reply.  You should not return with further anxiety-driven unrealistic questions.

1.  Persons who are undetectable cannot transmit HIV to others.  Persons can have undetectable tests, either because, if they have HIV they are on effective therapy, or they do not have HIV.

2.  Yes, persons can have recently acquired HIV can be asymptomatic.  Based on the history you gave, there is no reason to think that you have HIV.

3.  A person who had taken methamphetamine and participated in receptive rectal intercourse would know that they had been penetrated.

As I said in my initial reply, "You have no reason to be worried about HIV- the encounter you describe above is an entirely no risk situation.  It would not normally suggest a need for testing but if you are continuing to worry about the no risk exposure, testing may be a good idea to eliminate any lingering concerns you have about your no risk exposure.  Based on what you say above, I am confident that the test will be negative and that result will be conclusive."  That remains the most important answer I can provide to your questions.
