[Question #12234] Unprotected Vaginal with CSW

hi Doctor. This is my situation
  1. 10/18/2024 unprotected vaginal with comercial sex worker. Take 3 hour later 1 gram azitro and 800mg cefixime oral. Any symptoms at all. No any type of discharge compatible with gono or clamydia.
  2. 13/18/2024 unprotected vaginal with same person. Take 3 hours later the same antibiotics. 24 hours later in the morning (today) I have uretral discharge , white color to somewhat green. 
  3. No I’m in Emergency and taken rocephin 2gram indovenose. And Awating result from the discharge and results from blood sample.
My questions:
1. Having taken the oral antibiotics. Could this be Gono and Clam? This was a failure with the doses taken?
2. Im worried about my wife. From the first event and 15 hours later of the unprotected incident with CSW I had sex with my wife. If this is Gono , could this be from the first or the second encounter?

Thanks for clarifying the exposure dates. Your symptoms -- urethral (penile) discharge -- indicate you almost certainly have urethritis, i.e. infection/inflammation in your penis. Almost all urethritis is due to STD, and with colored discharge, gonorrhea is most likely but both chlamydia and nongonococcal urethritis (NGU) are possible. You did not acquire it during the exposure just one day ago; symptoms of gonorrhea cannot show up sooner than 24 hours after exposure, and usually it's 2-3 days for gonorrhea and 7-10 days for chlamydia and NGU.

So the source of your infection is a bit of a mystery. The exposure 4 weeks ago (October 18) was too long ago, and the antibiotics you took are very effective in preventing all three STDs. And as just noted, your most recent CSW event was too recent to be the likely source, although it might be possible.

Or maybe you were infected from your wife by sex with her in the last week or two. This could happen either if she has been at risk for STD, i.e. has other partners herself. You're the only one who can judge this, but I will point out that when one member of a couple has a need for sex outside marriage, often the other has done the same. Or if you were infected October 18 but the antibiotics were not effective, perhaps you infected your wife and have now been reinfected by her.

At this point, all I can advise is that you follow through with the doctor or clinic about their evaluation and your test results. The new antibiotics might make your test results negative, but usually not this soon; and I predict it will turn out to be gomorrhea with a positive test result. Whatever the cause, your wife is going to need treatment. Even if she isn't the source of your infection, she will need treatment on account of your sex with her soon after the CSW event.
