Hi Doc
I went to an escort service on 19-11-2024

I did not had sex, infact was wearing my clothes but she was naked, i helped my self ejaculate in a condom and did not had any kind of sex with her. 
I am worried about the following :-
1. She rubbed her right hand thumb on my lips twice or thrice and on my face gently, i am not sure if there was any active bleeding from her thumb as i can see a few cuts on it.

2. Should i start PEP as per the situation explained

3. Do you think this situation poses any risk for HIV transmission as if there was any bleeding from her thumb could come on my lips and further to my mouth

Welcome back to the Forum.  I would not recommend PEP for you.  The exposure you describe was a no risk event.  No one has ever gotten HIV from the sort of exposure you describe- you are not going to be the first.  As I said to you 10 months ago "HIV is transmitted only through penetrative sexual activity.  Receipt of masturbation, whether through body to body rubbing, a handjob, or with your hands after getting your partner’s genital secretions on them are all no risk activities.... There is no medical or scientific reason for testing and certainly not for taking PEP. "

HIV is not transmitted thought ingestion or contact of the lips with blood. 

This was an entirely no risk event, there is no need for testing and certainly no need for PEP.  EWH

Thanks a lot doc for your valuable advice, Since today morning i have 2 other doctors here in New Delhi and both of them assured of me that i do need PEP for the incident. 

Please resolve my following issues to finally put the matter to rest.

1. One of the doctor explained me that even if CSW had blood on her thumb and then she rubs her thumb on my lips and face will not pose any risk as HIV dies within seconds once it comes in contact of the AIR. Is it True? (We spent almost 5 min talking before she had put her thumb on my lips and face).

2. Can i with all confidence and no fear resume my normal sexual activity with my wife?

Your advice will put my mind at ease and will close this matter for good.

If you did not have sex, you do not need PEP.  I'm sorry the doctors you consulted are misinformed (unless your initial statement, that these doctors told you that you do need PEP and left out the word NOT)  In reply:

I agree.  That is the reason I feel that you do NOT need PEP.

you are correct I missed out NOT from the sentence, all my theory of being exposed was based on an assumption that she might have blood on her thumb which could have come in contact with my lips and further in my mouth and now after re assuring that it wont matter if it was or not another assumption took over me - "what if she used her thumb inside her vagina and then rubbed my lips and face with the vaginal fluid" please let me know if that will pose any risk of exposure to HIV. 

I again met the same doctor and told him about my mental situation after which i decided to go for PEP for which he took a written consent from me as they have already cleared me as safe from situation. Doc, please let me know the following :- 

1. Now that i have started PEP (42 Hours) when should i test again to re-assure myself that i am not infected. (i took a basline test today which was negative)

2. Have you known any cases who have taken PEP and didn't have any other exposure but yet turned out to be positive later.

3. Do I now really need to wait 6 months to be sure that i am not infected.

Your assumption is incorrect.  There was no risk.  Even if "if she used her thumb inside her vagina and then rubbed my lips and face with the vaginal fluid".   HIV is spread only by penetrative sexual activity.  Taking PEP is a waste of time and resources.  In reply to your final follow-ups:

1. Now that i have started PEP (42 Hours) when should i test again to re-assure myself that i am not infected. (i took a basline test today which was negative)
You do not need PEP.  Taking PEP is a waste of time and resources.  If you had a meaningful exposure (YOUR DID NOT!!!) testing would be conclusive 6 weeks after your final dose of PEP

No

3. Do I now really need to wait 6 months to be sure that i am not infected.
You need to address you insecurities.  you were not meaningfully exposed.  See my response number 1 about follow-up testing
