[Question #12320] Follow up

Hi, i hope you fine and in good health, i would like to update you on my condition, my symptoms 90% disappeared after 2 days from starting the antibiotics, i didn’t feel burning while urinating or that unbearable pain in my penis. The white discharge also stopped and turned into precum and recently that also stopped. There is some trivial pain but not bothersome. I educated myself on STD/HIV but i have few question to clarify because of different opinions from here and other online literature and books.
1- short brief less than 5 minutes protected vaginal sex 3 and half years ago. The condom didn’t break wide open, I couldn’t feel anything with the condom so protection was intact. Any risk here despite trivial symptoms?
2- i had frottage grinding with a woman 1 and half years ago, she was on top and started grinding luckily i was wearing a condom but when she got up the condom removed from my penis, either when she got up or during grinding. It was for 1 minute, im 99% certain i didn’t penetrate but that 1% is playing with my head ‘what if’. Any risk?
3- i have done quiet dozen handjobs with vaginal secretions as a lubricant, some were vigorously, they massaged the tip of the penis. Wouldn’t the virus goes inside the urethra when she was massaging the tip?
4- deeply fingered dozen of woman some with a cut or some not. One of the cut was an hour ago and i inserted it deep to the cervix, some of the occasion its was like fisting. Medhelp and Poz.com said air inactivates the virus but I didn’t remove the fingers like penile penetration. Any risk if slim?
5- how big is the wound to allow transmission? Is herpetic whitlow and sypillis chancre painful and obvious?
- it’s transmitted by unprotected sex but some says it can by the above activities.

Thats is why im little confused.
5- if the vagina fluids while she was giving me a handjob came in contact with the precum wouldnt that classify as fluid exchange?
6- Its was not gonorrhea from the swab of the discharge but i dont think its chlamydia either. I read NGU caused by unprotected sex but can it caused by non sexual penetration? Frankly i never had unprotected sex in my life, never know the feeling of penis touching or inside the vagina.
- i know im little naïveté but please bear with me . Thanks.

Welcome back to the Forum. On this occasion I will be answering. I have reviewed your earlier interactions with Dr. Handsfield and agree with the information he has given you. in doing so, let me reiterate several important points. Most commercial sex workers/masseuses do not have STIs. Even when they do, even with penetrative sex, most acts of intercourse do not result in infection. STIs, including HIV are NOT acquired through transfer of infected secretions on a persons hand in the course of masturbation. It sounds as though you may have gotten a skin infection due to trauma, self examination, use of a harsh soap or a combination of these factors. The skin of the penis is easily irritated. Your discharge and pain on urination were likely the result of the skin infection. I'm pleased to hear that the broadly active antibiotics you took have helped.
In response to your new questions:
1- short brief less than 5 minutes protected vaginal sex 3 and half years ago. The condom didn’t break wide open, I couldn’t feel anything with the condom so protection was intact. Any risk here despite trivial symptoms?
Condom protected sex is safe sex as long as the condom is worn throughout the sex act. An STI would no persist for3.5 years without obvious symptoms. I would not worry about this encounter
Any risk?
Frottage is a form of masturbation and a no risk sexual activity. EVen if her vagfinal secretions got on your skin (penis or otherwise, it would be no risk)
3- i have done quiet dozen handjobs with vaginal secretions as a lubricant, some were vigorously, they massaged the tip of the penis. Wouldn’t the virus goes inside the urethra when she was massaging the tip?
While I understand your concern, there is no known risk of this happening. Neither of us has ever seen or heard of such a thing in or combinated nearly 100 years of STI clinical activities and research.
This too is a form of masturbation. Even with a recent curt or scrape on your fingers there is no known/proven risk.
5- how big is the wound to allow transmission? Is herpetic whitlow and sypillis chancre painful and obvious?
- it’s transmitted by unprotected sex but some says it can by the above activities.
Non-infectious surface wounds appear to be associated with little or no risk of infection. the reason that syphilis and herpes are associated with increased risk is because, in efforts to fight the infection, large numbers of activated lymphocytes, the target cells of HIV, are present in the base of the lesions. These lymphocytes are the target cells for HIV and the presume reason for increased risk for infection associate with such lesions.
5- if the vagina fluids while she was giving me a handjob came in contact with the precum wouldnt that classify as fluid exchange?
No, the fluid exchange needs to take place at a mucosal surface. Accept the observations science that years of experience provide. HIV is just not all that infectious'
6- Its was not gonorrhea from the swab of the discharge but i dont think its chlamydia either. I read NGU caused by unprotected sex but can it caused by non sexual penetration? Frankly i never had unprotected sex in my life, never know the feeling of penis touching or inside the vagina.
The term NGU refers to urethritis (inflammation of the urethra) which is not due to gonorrhea. Sexually acquired NGU can occur doe to a number of different bacteria, including chlamydia. Your NGU was not due to sexual contact and appears to be due to a bacterial infection most likely due to skin bacteria entering at a site of inflammation
REplies will be limited. I hope these explanations are helpful. EWH

Thank you for the reply doctor Hook.
1. When i started to have symptoms before the antibiotics, i started to urinate brown- reddish urine twice and it was burning. Does that caused by STD?
2. You said transmission occur at the mucosal surface. During frottage penis- vagina, grinding against each mucosal surface, wont transmission occurs?
3- i had kinda like open laceration or cut on the tip of the penis, what it could be? Can chlamydia do that? Or NGU?
4- can chalmydia/ Gonorrhea or other STD transmit by handjob with vaginal secretions?
5- What kind of skin bacterial infection does those symptoms? Staph? And the open lacteration.
6- non infectious surface wound or cut causes little or no risk. How little doctor?
7- i know there is no proven cases but how come handjob with vaginal secretions massaging the tip and fingering deep with a cut is not a risk. Im worried due to doing dozen of times.
8- frankly doctor was i at risk of HIV and other fluids STD? I have a partner and im afraid i put her at risk?
I would like one final reply after this one. Please Thanks

I'm sorry that you are having trouble accepting that you were not at risk and did not have an STI. Brief follow-up responses:
1. When i started to have symptoms before the antibiotics, i started to urinate brown- reddish urine twice and it was burning. Does that caused by STD?
As I explained above, any site of irritation will burn when in contact with urine. Reddish-brown urine is not a sign of any STI. It can be a sign of a non-STI UTI or a kidney stone.
2. You said transmission occur at the mucosal surface. During frottage penis- vagina, grinding against each mucosal surface, wont transmission occurs?
As Dr. Handsfield and I have both indicated- NO!!!
3- i had kinda like open laceration or cut on the tip of the penis, what it could be? Can chlamydia do that? Or NGU?
The problem you describe could occur due to the friction related to vigorous frottage or masturbation. It would not be caused by NGU or chlamydia.
4- can chalmydia/ Gonorrhea or other STD transmit by handjob with vaginal secretions?
Repetitive. The answer will not change by repeating the question. No.
5- What kind of skin bacterial infection does those symptoms? Staph? And the open lacteration.
Staph and strep from the skin cause most bacterial skin infections but without a culture that would only be a guess
6- non infectious surface wound or cut causes little or no risk. How little doctor?
Sigh. Virtually zero. There's a risk that you will be struck by lightening while reading this too but it's not something to worry about.
7- i know there is no proven cases but how come handjob with vaginal secretions massaging the tip and fingering deep with a cut is not a risk. Im worried due to doing dozen of times.
You are arguing against the WHO, the CDC, and the experience of an entire field. I suggest you accept this fact rather than than argue. If reflects the mechanics of the activity , the biology of the organisms that cause STIs, and the dilution effect of transfer of material from one site to another.
8- frankly doctor was i at risk of HIV and other fluids STD? I have a partner and im afraid i put her at risk?
Another repetitive question. No
You have 1 follow-up remaining. After that the thread will be closed and further questions may not be permitted. EWH

1- i have being to urologist for a followup. He examined me and said everything is good and asked me when was the last time i ejaculated. I told him 3 weeks ago before the symptoms starts and today when i did, yellow colored sperm came out. Im worried about this. Its the first time.
2- The frottage happened 1 year ago so the open laceration or wound is not related to that.
3- i spoke with urologist about staph or strep skin infection. He said those are localized and wont cause discharge or burning or pain in the penis. Im confused.
4- I moved on from handjobs and fingering because hiv isonly transmitted through unprotected sex with fricition and ejaculating. Plus heterosexual spouse get them through repetitive exposure like negative husband and positive wife without treatment and some never catch it. Am i right?
5- although im 95% cured but im still having hard time accepting how did i got whatever infection from handjob and fingering only ( Mutual Masterbuation). I hope you can enlighten me more about it to officially move on from this because of urologist thinks it might be mycoplasma or ureplasama or chalymdia.
6- im sorry for this repetitive question and im anxious about HIV but you told me that its ZERO RISK of HIV from all the activites i have done and i didnt miss any details. I respect you and doctor Handsfield opinion and i spoke with an infectious disease doctor and he said he studied from your books and journals. I read your posts here of other people and you said there is little to no risk. Am i in the clear totally?
7- do i need testing on account of all these exposures?
8- can i safely do handjobs and deep fingering without worry about std and hiv?
9- can i resume unprotected sex with my partner without worry?
Thats all thank you.

This is a question for your urologist.
2- The frottage happened 1 year ago so the open laceration or wound is not related to that.
OK.
3- i spoke with urologist about staph or strep skin infection. He said those are localized and wont cause discharge or burning or pain in the penis. Im confused.
He has had the chance to examine you and I have not. Further questions should go to him
4- I moved on from handjobs and fingering because hiv isonly transmitted through unprotected sex with fricition and ejaculating. Plus heterosexual spouse get them through repetitive exposure like negative husband and positive wife without treatment and some never catch it. Am i right?
Correct. You and your partner are not a risk from masturbation
I have nothing more to say. Ureaplasma and mycoplasma are normally found in the urinary tract but occasionally can overgrow. Typically they do not need treatment
6- im sorry for this repetitive question and im anxious about HIV but you told me that its ZERO RISK of HIV from all the activites i have done and i didnt miss any details.
