[Question #12321] Pep and Hıv test

Hello
I had a male-male sexual intercourse on November 1, 2024. There was intercourse during condom. But I am scared it. 2 hours after intercourse, the Triumeq drug (PEP) method was started. I was tested 11-14-17-22-26-31 days after intercourse. P24 antigen + HIV1/2. The tests came back negative. 1) Should I test more? 2) Does the Triumeq drug have tested period effects? I also had the person I had sex with 13 days after intercourse tested and that person also came back negative for p24 antigen + HIV 1/2 antibodies. 3) What is my risk?

Welcome to the Forum.  I'll be glad to comment.  In our opinion, you over-reacted to the risk associated with the encounter you describe.  You did not know if your partner had HIV, condom protected sex is safe sex, and even if a condom had not been used, if your partner had untreated HIV the risk of acquiring HIV would be less than 1% if you were the receptive partner and less that 1/100th of 1% if you were the insertive partner. In our clinic we would decline to provide PEP for you.

Since then, your partner's negative HIV test proves that he was not infected and could not have given you HIV.

The PEP you are taking delays the availability of conclusive testing for HIV, not that you need it based on your partner's test results.  You cannot test conclusively for HIV while taking PEP and follow-up testing is not conclusive until 4-6 weeks after your last dose of PEP.  EWH

The World Health Organization emphasizes that even if Pep is used, tests done at 4 and 6 weeks after exposure should be accepted. In fact, some sources do not find it appropriate to take another test after a negative result that will be done and received 45 days after exposure.

Why did you say this?

The fact is that HIV PEP is therapy. Therapy can mask possible treatment failures. If you prefer to go with the WHO recommendations, please feel free to do so.  I am confident, however, that whenever you test, it will be negative since there was no need for you to take pep to begin with.

You ask for my advice and received it.  EWH

Dear Doctor, I am closing this topic by trusting what you said. There is no risk of infection and I was afraid for nothing?

I agree with that plan.  Thank you.  If you continue to use condoms for casual encounters you’ll stay safe.

Closing the thread now.  EWH
