[Question #12332] Drunken Night

Dear Dr
Hope you are doing fine
On 2 december 2024 early morning 245 am i had quite a few drinks and had an encounter with a csw. As far as i remeber the exposure was only receiving oral fingering and deep kissing. I remeber her being extremely wet. She also twice asked me to insert but i did not.
The whole episode lasted for about approx 50 minutes
Because i was so highly drunk and due to fear i feel i might have had unprotected sex but i did confirm with her also that same night and next day and she confirmed that i did not but my fear and anxiety is not letting me trust her.
also note not a great thing but after 40 minutes i might have cleant my nose with my finger and after that i felt there was slight blood is this a risk for HIV? Or any other form of std
Now after 3 days i feel very light sense of burning and want to pee is this a start of any std? There is no discharge as such
What std can i acquire from oral.
Can i conduct pcr rna hiv after 9 days
Please let me know what std tests should i do after how many days.
Regards.

Welcome to the Forum. Thanks for your questions. I'll be glad to comment.
We get many questions form clients who worry that during a drunken encounter they had penetrative sex with a partner when they did not mean to. In real life however, this is extraordinarily rare. Men are aware when they have penetrative sex and need to be awake to accomplish the act. In the situation you describe, I'll add that: 1. You do not think you had penetrative genital sex and 2) Your partner has said repeatedly that there was no vaginal penetration- why would she lie?
I should add that most CSWs do not have STIs or any sort and when they do, most single exposures to infected partners do not lead to infection.
As far as the other activities you describe, there is no meaningful risk for STIs, including HIV from masturbation and the risk from deep kissing also approaches zero. Further, there has never been a case of HIV acquired from receipt of oral sex- none!. As far as STIs are concerned, the main STI acquired from receipt of oral sex is gonorrhea and that is uncommon. Chlamydia is vanishingly rare and syphilis is likewise most unlikely.
I understand your desire to get the reassurance that a negative test would provide. To do this the most appropriate tests would be a urine test for gonorrhea and chlamydia at any time more than 3 days after your encounter. If you choose to have a blood test for HIV and/or syphilis, you'll need to wait at least 4 and preferably 6 weeks for a conclusive results (testing at 4 weeks would detect over 98% of recent infections). HIV RNA PCR tests are conclusive when taken more than 11 days after expsoure.
If you choose to test, I am confident that you will find you are not infected.
I hope this information and perspective is helpful. EWH

Dear Dr
Thank you for your kind reply. My exp was on 1st night 2nd early morning. I got tested for chlamydia trachomatis, herpes simplex 1&2, mycoplasma genitalium, mycoplasma hominis, neisseria gonorrhoeoa, urea plasma parum, urea plasma urealyticum. All negative test method muliplex r-pcr.
Urine sample was not given early morning randomly in the day. Is that ok?
Test period is ok at 4-5 days?
For hiv i will be testing with home kit at 24 days is that ok?
Any other test you think i should get done?

Your test results are reliable.
Testing for HIV is a bit more complex. Testing with a standard HIV antigen/antibody test at 24 days is a bit early. These tests are more than 98% conclusive at 28 days and entirely conclusive at 6 weeks. A PCR test for HIV taken any time more than 11 days after an encounter will be conclusive as well. However you test, I am confident it will show that you were not infected. EWH

Sigh. You have tested at a time when there are no good data to tell you what proportion of recent infections would be detected. That said, as you have already been told, there was virtually no risk for HIV from your encounter. I really did not think testing was "needed" to start with so obviously, I do not feel you need to re-test either although, admittedly, testing is always a persona choice.
This is my 3rd and therefore, final response to this series of questions. There really should be no need to return regarding this encounter. Closing the thread now. EWH
