[Question #12383] Need expert advice

Good afternoon doctors I am a gay male, and before these two encounters I tested negative for all stis. But beginning of October I had an encounter with male which involved, mutual masterbation, frottage rubbing of penis together, and sucking each other off. Second encounter, 3 weeks later, was a 3 some, which involved making out with one male, and also protected anal on him, and the other male only gave me unprotected oral as both gave me unprotected oral. 2 weeks later I get what I know almost for a fact is an outbreak, started having, lower back pain, leg pain, and redness, slight burn when I get, and when i shit it hurt alot for a few days( inhave never received anal) i got slight stinging and itchiness on penis. It went away, and about a week later, got the same or very similar symptoms. I saw some white stuff around penis, that looks weird, I assume herpes bumps, do not look textbook, but look odd. I went to a doctor to examine a few days later, and I did igg tests, it came back positive for hsv1 . I amNow going through what I believe is my prodome third outbreak in the past month in a half. My penis has been super red coming and going. I do not want to go through a full outbreak again, can you please help me out on what to do, i will obviously talk to my doctors as well, but

 1.seeing you guys are top notch what do you usually recommend for ghsv1 since its not two what medicine should I take? I have also had redness on chest a few days, and a bit on body, neck, could this be due to the herpes, or possibly hiv? I have body aches that come with it, but no sore throat lr cough?3.since i aquired herpes how much more likely is it I acquired hiv, I heard it makes it more likely, I am super scared. 4.should I be taking valtrex? Or any form of suppression therapy? I don’t want to keep dealing with the symptoms 5) I have a new partner can i pass hsv1 easily to them with unprotected sex? How about if they give me a blow job, they tested negative for both hsv1 and 2 6. What is disclosure form if you will, since it is hsv1 or not two or does the strain not really matter? 7) can you tell me about natural remedies such as lysine as well

Good morning, while it is possible that you have acquired HSV 1 at these encounters, it is unlikely because if this was a new infection,  you would not yet have antibody, which you do, because your IgG was positive for HSV 1. 
The medication for HSV 1 and HSV 2 is the same, either acyclovir or valacyclovir. 
However, I suspect that this is not a herpes outbreak but rather something else.  You are reporting generalized redness and other symptoms that don't necessarily represent herpes.  When you were seen by the doctor, did they do herpes swab testing from your penis?  Did the doctor think you had herpes?  Swab testing would have helped to know if this is herpes or not.
If this is herpes, again, it is not new since you already have antibody.  Antibody comes after you've been infected for a while, it is not there right away. 
In terms of partners, you could disclose that you have the cold sore type virus, but you don't know if you have this orally or genitally.  You don't know where this is located as again,your symptoms are not necessarily consistent with HSV 1. 
If the doctor prescribed antivirals for you, you can certainly take them, there is no downside to taking them if you do not have herpes.

Hello gHappy new year, the test was almost about 9 weeks post encounter wouldn’t that be enough time? Also by the time I went to the doctor it was a bit late, so I will need to wait for the next time they appear. They lasted about 4 weeks, but sith holidays, also was just too crazy to get appointments also. But I am 99% sure it’s herpes way too many symptoms. So Terri I forgot to mention I am bisexual not just fully gay. I am now dating a new girl, we have had condom protected sex about 5 times already, she got tested for everything a month ago, including herpes, and everything came out negative, so I know she was herpes free. Can you please talk to me more about my chances of trasnmitting it to her? Up until recently had no idea that hsv1 was able to be on genitals, and also that it was less likely to be passed over hsv2? 3 i’ll put three since I asked a few stuff already, but what are the chances that she gets it, if She touches a sore and then here genitals, or If I touch a sore, and then finger her. 4) she did this the other day and now has been complaining about lower, back , calf, and foot pain. Stomach pain I have also experienced it , so am now freaking out that I have it?

5) what is the chances that she gets hsv1 orally by giving me oral to sores on my penis? Or would it go genittaly to her as well? (She was hsv free both types before our encounters) 6) chances of her getting it genittaly, if we keep using condoms, every encounter has involved condoms7) should i take antivirals to lower risks? Is that something that should be taken everyday for hsv1 genittaly 8) i just remembered I also gave her oral about a week ago, what are the chances she also got it this way? She has mentioned all week a bunch of symptoms about lower back, calf, knee, and now buttocks pain, and headache, nausea, and now she feels hot, besides the feeling hot part, that is how I started to notice my symptoms as well, any advice there, I feel she is going through prodome I am freaking out

TRansmission of HSV 1 during intercourse is very rare, I've personally not seen it happen but it is possible.  Especially when someone has a new infection with HSV 1, which you do not have, due to the presence of antibody in your blood sample.  But please remember that you don't know that you have this genitally.  You described the bumps as not consistent with what you see herpes looks like normally.  
Condoms reduce transmission of HSV 2 from male to female by 96% when used regularly for intercourse, so that statistic should certainly hold true for HSV 1 as well.  
Because we don't really know the location of your established HSV 1 infection, I can't say that giving oral sex is risk free - you could be infected orally instead of genitally.  
Antiviral medication may reduce the risk of transmission to your partner, and certainly wouldn't hurt to take it if you and she agree about this additional step.  The medicines are safe and effective.
Her symptoms don't sound like prodrome to me, however.

Thank you for the quick response, Terri I think you missed the most important part in it, and I say most important because this is the one thing I looked all over, and can’t find the answer on this thread, seems many don’t ask it. I now have more stuff on my penis forming, I have now had more redness and white looking stuff ln my penis which seems like another outbreak, I am calling today to make another appointment, but with that being said, if these were active sores that I currently have 1) what are the chances If the girl touched my penis(basically stroking and touching the sores, and immediately touched her vagina and rubbed it, in less than 5 seconds that she could acquire the herpes herself? 2) also vice versa, if I touched my genitals on active sore and then hers in less than 10 seconds without wahsing hands (Btw this isn’t hypothetical this ,
Literally happened last  week, and is one of my biggest fears, and reasons I think she may have caught it,  I can’t find any answers on this scenario on this thread.
3) I know you guys hate the igg test, but if I take another test this Friday, and it comes out negative for hsv1 and 2  (it would be 12 weeks after 1 of the encounters, and 11 weeks and 5 days , would that be close to conclusive I don’t
Have them or at least more so hsv 2 , since I heard it is 92% after 12 weeks? What is your take, I just at least wanna be 100% sure the hsv2 won’t change, since the other test seemes too soon.

I am going to get this swabbed tested this week, so if it comes back negative, I know I won’t have to come back to the thread and ask another question, if it comes out positive, I will be back for follow up, if it is indeed determined it is genitally, so thank you for the responses

That is a very low risk but  not totally impossible.

That would be 92% accurate, yes

Nothing is 100% but in all of the patients that I have diagnosed with new HSV 1 genitally, ALL had received oral sex in the past two weeks.

While that is possible, it is rare.

Do get this swabbed right away to be more certain
