[Question #12397] Exposure Risk

Thank you for this service. I had protected intercourse with a CSW that was brief and ended when we realized she had started her period. The condom remained fully intact however there was blood on the condom and the base of my penis that was not covered. I do realized that I was most likely protected, however, I have HSV2 and wonder if that should be a concern. I have taken suppressive medicine for years and have not had a breakout in many years. Should I be concerned about HIV and consider PEP? I am nearing the end of the 72 hour window and greatly appreciate your guidance.

Welcome to our Forum and thanks for your questions.  I'll be glad to comment.  Congratulations on your safe sexes practices as well.  That your partner had started her period does not change your risk for HIV or any other STI. If your partner was infected, which is unlikely, the amount of virus in menstrual blood is no different that the amount in genital secretions and even if her blood/genital secretions got onto the skin at the base of your penis, there would be virtually no risk for HIV.  I would not worry, I certainly see no reason to seek PEP (and would not prescribe it if you asked for it in my clinic) and really see no scientific reason for testing.  Your HSV-2 infection does not change this assessment.
