[Question #12403] Fingering Risk

Hi doctor,

I am a 25 year old women, a guy fingered me while I checked on his fingers looked like there was a  some open wound as he bite his fingers alot. Would this cause any HIV risks if there was blood from his fingers, healed wounds & dried blood. He could be HIV positive looking from his previous relationships (MSM). Although the fingers didn't went all the way in.

Welcome to the Forum.  Thanks for your questions.  I'll be glad to comment and hope that you will find the answer reassuring..  Masturbation, including fingering is an entirely no risk activity in terms of risk for HIV and all other STIs. This is the case even if your partner has a fresh cut or scrape on his fingers- still no risk.  In addition to our confidence in this matter, this is also the assessment of the CDC and WHO- no risk, no need for testing, no need for concern.

Hi doctor,

Even though there was blood present on the finger ? It is still no risk event ? What is the reason behind this as I thought any amount of bodily fluids could mean transmission. Can I be confirm that even any blood was present I am 100% safe ?

Correct,  no risk even if blood was present on his finger.  This sort of surface contact does not transmit HIV or other infections.  No change in my assessment or advice.

Hi Doctor, just got back from meeting the same guy

He was on PEP due to a risky exposure, tested negative PCR RNA 11 DAYS & ALERE EARLY DETECT FINGERPRICK 3 WEEKS POST PEP. 

HIS PARTNER TESTED NEGATIVE 45 DAYS AFTER EXPOSURE (However blood test was done without being fasted).

1. How much blood is needed during fingering to pose a risk ? He confirmed that his fingers may have some blood due to peeling his skin before our interaction.

2. Is his test any conclusive ? 

3. I have read that infectious period and conclusive testing period differ ? How does it differ ? Does it mean that if a person were to test they would have been positive much earlier and conclusive testing period is to confirm that HIV is confirm not present ?

3. His partner tested after 45 days of exposure, is his test conclusive ?

4. The partner tested 4th generation CMIA TEST does these test at 45 days could it turn positive after ? Are these test often for false negatives ? 

5. Do we need to be fasted and no food or drinks before taking HIV test ?

Why I am asking on the fingering risk is because I am afraid that he could be newly positive due to the window period and his little blood on his finger could pose a risk due to high viral load ? I am still in the 72 hours mark to take PEP .. hopefully doctor reply ASAP

Your clearly anxious about this event.  You should not be.  If he is on PEP and has had a negative HIV PCR test, there is no way he could give you HIV even through unprotected penetrative sex and and as I've already said, masturbation of the sort if NO RISK, even if he had HIV which he has proven he does not.  This will be the final response.  There should be no need to return following this reply:

You missed the point, even if fresh blood were present and he was infected, there is no known risk.  None

His test proves he does not have HIV and could not transmit it to you.

I don't know where you heard this.  Persons with conclusive results cannot transmit HIV.  This is a meaningless distinction.

Yes

Repetitive.  This result is conclusive.  This person does not have HIV and cannot transmit it

No

What are you referring to surface contact ?
Surface contact refers to touching genital secretions or blood from anothr person

Relax.  You do not need PEP.
