[Question #12420] DGI possibly?

Oct 22 a sex worker gave me unprotected oral sex for 10-20 seconds. I had no symptoms of any STD. 4 weeks later I had urine and blood test come back negative (chlamydia, gonorrhea, syphilis, HIV). One week later I had unprotected sex with my partner. Within 5 days she developed a rash on her neck. She also developed hives on her leg with a painful red swollen ankle. Her rashes went away but then joint pain started. She had trouble with her joints in her hands for a while. Currently she has deep pain and mild swelling in both ankles and one knee. Her heels also hurt. Most of the pain occurs late at night/early morning. Is this DGI?

There is no evidence that you have penile gonorrhea. If you do not have gonorrhea, you could not have given it to your wife. If you had DGI your urine test would have been positive. Please don't worry. EWH---

Thank you doctor. Your reply has set me much more at ease. I believe I know the answer to my following question, but please bear with me. How likely (or unlikely) are false negatives in the urine testing I exercised? I attended a US county sexual health clinic. I continue to worry as I feel my partners symptoms align with much of what I find from various health resources.

I think you are misinformed on both the symptoms of DGI and the causes of the rash and joint pain your partner has been having. A wide variety of things can cause symptoms of the sort you indicate she is experiencing. STIs do not figure prominently on that list.
Urine tests for gonorrhea could miss 1-1.5% of active infections. A negative test, when you combine both the fact that most people DO NOT have gonorrhea, the inefficiency of transmission through oral sex and your negative test, the likelihood that you have gonorrhea that was missed by your test is a very small fraction of 1%.
Your partner should see a doctor who can assist her, you need to support her, not be consumed by any guilt that you might feel. EWH
