[Question #12451] HIV ? Rubbing ?

On December 27, 2024, I met a sex worker from Morocco. I’m 27, and after kissing her while drunk, I told her I didn’t want to have sex. We agreed on rubbing, and I made sure not to penetrate. She suggested I put on a condom, but it fell off because she wasn’t wet. I continued rubbing and checked frequently to ensure I wasn't inserting. Afterward, I wrote a memo to reassure myself that I hadn’t penetrated her.

The next day, anxiety set in, and I called her. She assured me I didn’t penetrate her and that she would never allow it. However, I’m struggling to believe her. What do you think the risk of getting HIV is?

She mentioned she gets tested every month and showed me her last test from December 13, which was negative. Should I ask her to test again? Given all this, should I be worried about my risk for HIV?

Welcome to the Forum.  Thanks for your questions.  I'll be glad to comment.  The encounter you describe was virtually no risk (body to body rubbing of the sort you describe is called "frottage")

The fact that your partner is tested regularly and was willing to show you her results is reassuring.  I would tend to believe her, both because most people tell the truth when asked, because you saw her record of testing and finally, because if penetration had occurred you would be aware.  

My advice is not to not worry about this encounter.  I see no medical reason for testing although I do acknowledge that many of our client's choose to test for the reassurance a negative test provides.  EWH

Thank you so much Doctor for answering my question, it’s just I had this anxiety about hiv  I was very sure back at time I don’t penetrate but somehow now I don’t trust my self and started to question myself she even told I was not that drunk to not remember what happened 
She told that I’m being over thinking and she sympathises with me and because that she agree to show show the last result to me 

1 - so even if she hiv + I have no risk from frottage ?
2 - can I test at 4 weeks 4th generation and move on from this event ?
3 - do I have worry about other STDs ?

I remain confident that no penetration occurred based on the description you provide.  In response to your other f/u questions:

1 - so even if she hiv + I have no risk from frottage ?
Correct

2 - can I test at 4 weeks 4th generation and move on from this event ?
If you choose to test, at 4 weeks (28 days) a 4th generation HIV test will detect over 90% of recent infection.  Results are not 100% predictive until 6 weeks 

3 - do I have worry about other STDs ?
Not realistically

Sorry, Doctor, for coming again, but anxiety and fear are interfering with my daily activities. I'm preparing for Step 1 and can’t focus since this encounter. I'm asking her to take a test in a few days; her last test, two weeks before our encounter, was negative for Ag and Ab. 

1. If I penetrated her for seconds and she tests negative 10 to 20 days after our encounter, am I out of risk completely, even if she was in the window period? You mentioned that a person with a negative result can’t infect someone. Does this apply during the window period? Statistically, it’s unlikely she got infected, but I need clarity. 

2. I know a person becomes highly infectious at the beginning of an infection. Are they contagious before lab results can detect Ag and Ab?

As you know, this will be my final response.

1.  You stated you did not penetrate her.  If you had, you would have known it.  In fact, you described the measures you to to verify that penetration did not occur.  Don't let your anxiety confuse you.  That said, if her test at 10-20 days, there is simply no way she could have infected you.  

2.  When persons have recently acquired HIV, as you point out, they may have very high viral loads which, in turn mean that the p24 antigen is detectable.

You are worrying needlessly.  I encourage you to accept this and move forward.
