Hi Doctors and sorry to return. This will hopefully be my last line of questions. I also asked Nurse Terri ok her site so I'm hoping to get a second opinion from you two. I understand her position on this as I've seen many replies to post almost identical to mine. Given my sexual history and my most recent partner, I took an HSV 1 and 2 test. Hsv was positive and high number. HSV 2 was positive at 2.45. The test was a Quest Diagnostic Test with inhibition ordered. After the results showed a 2.45 I got another message below.
The HSV-2 IgG screening assay was repeated on a different platform as part of the inhibition test, and the result was negative; thus the Inhibition result could not be determined. Most samples exhibiting this type of discrepancy have initial HSV-2 IgG index values of 1.10-3.00.
Can I assume it's a false positive? nurse Terri says these are usually good and don't require WB,.

Forgot to add my partner was negative for HSV2 consistently for years and most recently as well after 2 years of no sex. I took the test at 11 weeks and 4 days. Aside from my wife who doesn't have hsv2. Before that it's been 6 years since sex and condoms used. Never had symptoms of heroes on genitals. Only cold sores. I took aciclovir for about 4 days during those 11 weeks for a cold sore. Before that I would only take when an outbreak which is maybe once a year. Now I'm taking for suppressive out of precaution that test may not be false positive. ,

As you know, Terri answers questions on the Forum regarding HSV while Dr. Handsfield and I answer questions on other STIs. Other than the fact that when available (and she is), Terri takes virtually ALL questions exclusively dealing with herpes, clients are not permitted to request who gets their questions, nor are duplicative questions encouraged. There must be a misunderstanding somewhere along the line here. An index value of 2.45 may well be a false positive and requires a western blot for clarity. Especially given your sexual history. Please consider doing that either with your own provider or through me at westoverheights.com.

So for more clarity on the situation. 5 lifetime partners. 2 out of the 5 showed me HSV2 negative test. The most recent and the one 5ish years before her. I have always been tested as well as partners for STD's. Always negative, but never tested for HSV2. Never had symptoms. From everything I see at Westoverheights, you mention that the follow up test for the inhibition have excellent agreement with the WB and people do not need WB. The Urologist and doc who saw me mentioned the same. Without symptoms and given the index value and inhibition being negative, odds are false positive. Exception of my wife I have had very few unprotected encounters. I took test to cross HSV 2 off the list of STl. I still keep in touch with all the women Ive had sex with and none have had symptoms ever. Your comments to others who posted the same verbiage "test being done on a different platform as part of the inhibition and being negative is a great sign since the agreement has been excellent". I assume I would have had years to develop antibodies for HSV2 had I had it and would have had at least an initial outbreak or one recurrence by now given all the info that I have seen from you docs. You mention IGG test are as good as they get after 8-10 weeks and ive also seen that 5 days of anti virals would not affect test for initial infection. Married 18 years. We took a break during those years which is when I had others. Minus my affair 4 months ago. Before that the sex I had was with a girlfriend in high school. The most recent partner was negative for everything, had been sexless for a little over 2 years and had years of negative hsv2 test.
1. Is the WB necessary or can I believe the inhibition results and move on?
2. If I decide to do the WB, how long do I need to stop acyclovir?
3. How much is the Western Blot today?
4. Should I discontinue sex with the wife?

You remain hung up on a single encounter which occurred 4 months ago. The risk of acquiring HSV (or any other STI) from a single encounter is very low. Your test results with an inherently less than perfect test are equivocal. If you want to find out for sure if you have HSV, the best test you can get is a Western Blot performed at the University of Washington (beware, some labs offer Western Blots that are not the gold standard, Western Blot from UW). Should you opt for the WB and it shows that you have HSV-2, that does not tell you how long you have been infected and, in the absence of HSV symptoms, it is as, if not more likely that a positive test reflects prior infection than your single exposure to a casual partner of unknown duration 4 months ago. I am not at all sure how that helps you in the least. Thus, in response to your questions below:
1. Is the WB necessary or can I believe the inhibition results and move on?
This is a personal choice. My advice would be to move on. I see little to be gained from further testing.
2. If I decide to do the WB, how long do I need to stop acyclovir?
Taking acyclovir at this time will not impact your WB results. The WB detects antibodies, reflecting past infection. ACV does not impact the WB
3. How much is the Western Blot today?
I do not know. The blood would need to be drawn, processed and sent to UW where there would be the costs of the test.
4. Should I discontinue sex with the wife?
Absolutely not. She's been exposed for the past 6 years. You don't know that you have HSV or if you do how long you've had it, and it's quite possible that your "positive" test was falsely positive.
As you know, this will be my final response. I urge you to move forward without continuing down the guilt-driven "rabbit hole" you descended 4 months ago. EWH

Thank you both for all the input. Based off your comments, this forum, Nurse Terri and her forum, as well as my own doctors, I will take this as a false positive which seems to be the most likely outcome. Nurse Terri mentions in her Westover Heights forum that the follow up to the inhibition test with Quest has excellent agreement with WB. With all of this information available between this site and hers, the never having symptoms in me, wife, or any partner, I will drop the worrying and will stop testing. If I ever feel the need to I will work with Nurse Terri on her site to get it ordered. I know I have been a pain and I truly do appreciate the patience with me. All 3 of you are an amazing resource to have and I wish all of you a fulfilling New Year! Have a great day!
