[Question #12486] Receptive oral and incorrect prep use

Hi Doctors,

I am a male in an committed relationship with a female. I unfortunately had an experience with another man I regret last night.
The encounter was mutual masturbation (penis rubbed against each other, and abdominal area). I sat on top and his shaft made contact with mi perineal area.
Only received around 15 seconds of unprotected oral sex. I did not give any oral. Encounter was at 10pm.
Earlier, at noon I took I dose of on demand prep (1 single pill) and to my dismay I realized just now I should have taken 2 (2 1 1). I have just taken 2 doses now (will be 1 2 1). I am also hpv vaccinated and did not see any obvios lessions on his penis nor mouth, and he has hiv and siphilis negs from November.
Assuming I will test for gonorrhea in the next few days and take doxy pep:

Am I at risk for HIV? Will prep still work even if I took it incorrectly? I am really freaking out as I realize it may do nothing as I did not take it correctly.

Will it be safe to resume unprotected sex with my wife after this event and test results?

Finally, I just took the final dose of a 7d levofloxacin treatment for a stomach bug (last was around 3pm, before encounter), would that offer any level of protection against the bacterial stis?

Welcome the forum. I'm happy to address these questions.

You overreacted to an event that carried no significant risk for any STD. I certainly would not have recommended post exposure prophylaxis (PEP with a PrEP regimen). HIV has never been known to be transmitted by oral sex (mouth to penis) and body rubbing or external contact of genitals with skin, anus, etc is also risk free. So is hand-genital contact, even when genital fluids or saliva are used for lubrication. I would also advise against doxy-PEP, which is recommended for unprotected anal sex between men but not the sorts of exposures you had. You really don't even need testing for anything; your wife is not at risk for any infection from the events described.

Those comments cover the first two questions:  you didn't need PEP.  The effectiveness of the PrEP regimen taken after instead of before exposure isn't known, but since you weren't at risk for HIV, this doesn't make any difference. (For further reassurance, why not contact your male partner and ask about his HIV status?) And yes it is safe to resume unprotected sex wit your wife, without waiting.

Levofloxacin is highly effective against chlamydia and against roughly half of gonorrhea strains:   being on it when this exposure occurred definitely prevented chlamydia and reduced any gonorrhea risk by half. But since these were't risks anyway, this also doesn't really matter.
