Dear doc, sorry to continue. I was assured from your reply but just need clarification of your last sentence. You said ‘recommend this more for the assurance it will provide that any concern that you have acquired HIV’. Did you mean recommend this more for the assurance it will provide than any concern that you have acquired HIV. Your use of word that instead of than confused me. I hope you have no concern of me being hiv positive. I hope you meant to use the word than. I also need to know unequivocally if you feel the need to be retested. I would be assured by that and would choose not to retest if you say you don’t think there is a reason to test. Please state so in clear and unequivocal terms as u am only relying on your expert advice. Will not question again if you clearly give advice regarding testing again. Only your opinion matters and i will control my anxiety based on your expert advice. Sorry for being at it so much but I truly hope you would help. This question is for Dr. Hook as I was consulting him.

Your anxiety is out of control.  I am confident you had falsely positive tests, most probably for the HIV p24 antigen.  I suggested testing only for the reassurance that either of the two suggested tests would provide when negative.

I cannot tell you unequivocally to test or not- testing is always a personal decision.  Try to relax.  EWH

Dear doc, as I have read in some of your replies that oral sex rarely results in hiv infection. In my case, where i could have licked fresh blood from the deep cut on the forehead of Sex worker by licking, you said it is no risk. Is there no risk if infected blood from a wound directly enters my mouth? Would that blood not infect me by virus going in my mouth directly? Do you think of any chance of hiv transmission considering my facts?  In your opinion, is there any chance of me being infected by hiv? The CSW( trans ) comes from a high hiv rate state in my country. Not able to take the positive 4th gen test results out of my head even though the 21 day RT PcR RNA test being negative. Sorry for this again but I am confused. Is my case really no risk?Her wound/ cut was on the forehead and u was licking.

One small detail. My RT pcr Rna was a quantitative hiv test and not a qualitative one.

This is a repetitive question.  Repeating your question will not change my assessment.  As I said above, ingestion (swallowing blood after licking a cut on an infected person is a virtually no risk event.  

You should believe the results of your quantitative HIV RNA PCR- testing with either a qualitative or quantitative test will provide accurate results for you.  EWH

Thanks for the reply. My last question, what if all blood was not swallowed and some remained in my mouth and/or on my tongue. Would that transmit hiv to me? Moreover, if the viral load initially transmitted to me was low, would the quantitative RT PCR RNA test still detect virus in the blood by day 21 after exposure(The test mentioned min 40 copies of RNA can be detected by the test). 
Thanks in advance for your patience and expert advice. I am also sorry for so many questions, at times repetitive( not like this usually got really scared as u did something like this for the first time).

One small addition- I have lost around 3-4 kg weight in the past 15-20 days. And as you can see by my posts, I have been extremely worried and anxious and have been eating less than normal. Would you still say I do not have hub as you said in your first post accounting the weight loss as well.

this will be the final reply.  There should be no need to return to the Forum regarding this encounter or your test results.

My assessment is unchanged regarding whether or not the blood remained in your mouth.  Still no meaningful risk

When persons have untreated HIV their viral loads are typically in the thousands or tens of thousands.  The reason the limit of detection is low is because that is effectively zero.

I suspect your weight loss reflects your anxiety.  My assessment remains unchanged.
