[Question #12506] HIV and STD transmission

Dear doc ,

I come from China.Through  study  I know the basic fact about STD(including HIV) transmission is  that it generally requires a bare penis (no condom) entering another person's vagina, rectum or mouth.But recently I got HSV-2,so I have two questions.

1. Above-mentioned the basic fact Is it just for people who don't have STDs or everybody(including STDs Infected person ) ? If I have HSV-2 or another STD and I am from China, would it still apply to me?

2.Through  study  I know HSV-2 infection is associated with enhanced sexual acquisition of HIV,and the area I was infected with HSV-2(lesion) is the base of my penis, where the condom cannot cover, and often can cantact with infected secretion . If the cutaneous sites of recurrent outbreaks in the future is still at the base of my penis, is it possible that even sexual intercourse with condoms ,I still can be infected with HIV or other STDs from the  irregularity skin lesion contact with infected secretions ?   Sorry, my English is poor. I don't know if I've made myself clear.

Welcome to our Forum.  Thanks for your questions, as well as your confidence in our service.  I'll be gla to try to help.  I'll go stright to your questions:

1.  Having genital herpes does not change the way that HIV can be acquired- the routes of sexual acquistion are through unprotected genital, and or very rarely, receptive oral sex with an untreated, infected person.  Persons with HSV-2 are at somewhat increased biological risk for acquistion of HIV if exposed but the chance of infection remains quite low.

2.  Contact of infected lesions at the base of your penis through the sexual activities mention above can lead to infection but again, the increase is risk is modest.  Simple contact of a lesion at the base of your penis is not considered a risk.  If your initial outbreak was at the base of your penis, that is the location where recurrences are most likely to occur.

I don't want to think too much about biological reasons ,so even though I had HSV-2 and the initial outbreak was at the base of your penis,avoid STD(including HIV)enter my life ,what behaviors I need to avoid?

While herpes slightly increases the risk of acquiring HIV if exposed, the overall risk remains relatively small.  Decisions regarding the level of risk you wish to accept are a personal choice.  General recommendations to reduce risk include consistent condom use and asking partners if they are infected or have been checked recently.  For those who feel themselves to be at high risk, taking pre-exposure prophylaxis (PrEP) with anti-HIV medications to prevent acquistion of HIV reduces risk greatly.  Recommendations for PrEP can be found at the US CDC or WHO web sites.  The decision to take PrEP is a personal choice which should be made in the context of discussions with your own health care provider.  EWH---

Thank you for your patience,last question. Even though I had HSV-2 ,If I don't have unprotected vaginal or anal sex with new or casual partners,and don't share drug injection equipment with other people,  I will not be infected with STDs (including HIV)?

If you do not have unprotected vaginal or anal sex with new or casual partners you will not be at risk for HIV, gonorrhea, chlamydia or trichomoniasis.  Even without penetration the is a very small, mostly theoretical risk of acquiring syphilis if you had direct contact with an infectious lesion.
