[Question #12511] HIV Transmissibility and testing correlation

Hello doctors. Is HIV transmissible after 8 days? Then to add on to that if you had sexual contact with someone 8 days after possible exposure than did an RNA test 3 days later (11 days after exposure) assuming you had the virus and it was transmissible would that guarantee a positive result? As in would an RNA test become positive as soon as you’re transmissible?

Then also what are the current definitive conclusive timelines for hiv testing? Ive seen conflicting information online and even on here (not in any way your faults new information and data are constantly arising ). Ive seen on here that RNA testing isn’t conclusive and needs a confirmatory 4th gen test, that its conclusive at 14 days, more recently that its conclusive at 11 days and then online the CDC says its conclusive at 33 days. Then even for 4th gen i’ve seen some health guidelines say 28 days, some 45 and on here both. Lastly not to sound tedious or stupid but when an expert says something is conclusive “after” X amount of days does that mean you should test a day later or on that said day?

Advice changes over time as new data become available. Our previous advice about confirming negative RNA with later AgAb test stopped almost a year ago. We now advise that RNA testing alone at 11+ days is conclusive.
HIV cannot be transmitted if RNA is not detectable in the blood. It is always detected by 11 days, but sometimes earlier. The timing of RNA positivity and potential for transmission probably are identical.
Future anxiety driven questions WILL be deleted without reply and without refund of the posting fee. Experience has taught us that repeated questions tend to prolong anxiety rather than improving it, and ASHA is not keen on accepting fees for previously discussed questions or those with obvious answers. Such questions also have limited educational value for other readers, one of the forum's goals.
