[Question #12533] Antibiotics and safer sex

Dear Doctors,
I am back with another antibiotic-related question.
I recently had a small procedure done and I am currently on 400 mg of Cefixime for the next 5 days post-operation.
That said, I would like to explore rimming a CSW. I know performing analingus mostly carries risks for garden variety bacterial infections.
My questions are:
1. Would I technically be 'protected' from these bacterial infections, if I am on 400 mg of Cefixime?

2. If you had to provide an estimate of the risk, how likely am I to catch anything from performing a rim job, bacterial or otherwise? I know the risk it quite low, but I have seen ballpark estimates as low as 5%.
I hope you do not mind the novelty of my questions, but I am always looking for ways to stay safe.

Welcome back to the forum. Thanks for your continued confidence in our services.
1) Cefixime is highly effective against gonorrhea and syphilis bacteria and would be 100% protective against both. It has no effect on chlamydia, but oral chlamydia is so rare that it really doesn't matter; you probably would not be infected even if a rimming partner has rectal chlamydia. Cefixime has highly variable effectiveness on the various bacterial causes of intestinal infection, like E. coli and related organisms -- on average, probably would provide roughly 50% protection against such infections. (But these infections are low risk to start with.) The main infection risk from analingus is viral gastroenteritis, for which (as I believe you know) no antibiotic provides protection.
2) No data exist on which to estimate the chance of infection by performing analingus (rimming). The STI risk depends mostly on the nature of your partners -- but as we may have discussed previously, the large majority of female escorts (expensive sex worker by appointment) are disease free at any point in time and your risk probably would be well under 5%. If you're having sex with men, STI risk is far higher than with most potential female partners. As I said above, in all cases the main risk probably is for viral gastroenteritis.

1. Does how long I take the antibiotics for matter? (i.e. if I took a single-dose of 400 mg of Cefixime, post exposure to performing a rimjob, would that provide adequate 'protection')

FYI, the follow-up comments windows are offered for the purpose of clarifying the questions originally posed, not to raise new issues. That said, this one certainly is valid in follow-up to your first question above.
You don't say how long post-exposure, but if within 24 hours it was definitely effective -- and probably if within 72 hr.

I'll use my last follow up question to clarify one thing. Does this apply to a single dose of Cefixme 400 taken within 24 hours of performing rimming?

Yes, as I said above. I haven't changed my mind since yesterday! But be clear that I do not recommend cefixime (or any other antibiotic) in this situation. That's been your decision, not my advice!
You're welcome to post a new question in the future if substantially different than your three questions so far. But your sexual exposures and decisions seem pretty much the same over all those discussions, so it might not be necessary.
