[Question #12542] Fear and anxiety hiv

I feel ashamed to reach out again, but I'm struggling and worried I've ruined my future. I just graduated from medical school and asked a CSW to take a test, which will be either 24 , 27 days since our exposure. Does this make difference? I thought if she tests negative and I take mine in 28 days, there’s no way she could have infected me. However, I read Q#9500 and understand there’s a possible way to transmit the virus even if tests are negative. U stated on another question that a person whose HIV test are not yet positive can’t transmit the infection?
She sees clients every other day and has been doing this for five years. During our encounter, I asked if I was penetrating her, and she said no. Also there were no vaginal secretions on penis as she was dry I was anxious and drunk but aware of my actions. I worry my memo after the incident may have been misleading because I was drunk.
Sorry and thanks for understanding.

You've apparently changed your user name so I cannot use any prior posts for context. Thus I'll be working with one hand tied behind my back.
I think you are over reacting. Most CSWs do not have STIs, including HIV and most exposures do not lead to infection. Your partner told you there was no penetration and should be believe- if there was no penetration, there is no risk, even if you came into contact with vaginal secretions. Further, transmission of HIV by persons with a negative test is very rare and only occurs within a very few days of having acquired the infection. If your partner's test is negative at 24 days of more after your encounter and the test is negative, she cannot have transmitted HIV to you. Further, your 28 day test result will be more than 98% conclusive- we have never had a client with a test which is negative at 28 days go on to become positive.
I think you are worrying far, far too much. EWH

Thank you so much Doctor for responding to my questions I didn’t change my username and the previous question number was :
#12451
1 - “ If your partner's test is negative at 24 days of more after your encounter and the test is negative, she cannot have transmitted HIV to you. “
Even if she has the infection but it’s not detectable?
1 - so if her tested at 24 days and she is negative then my risk is zero ? Even if there was a brief penetration? And do I even need testing after her test ?
2 - should I tell her to wait till 28 or there is no difference ?
3- if her tested negative at 24 or 27 and mine was negative 28 days post-exposure do I need further testing ?
4 - I know the risk of frottge and kissing is zero but I’m afraid of penetration am I right ?
I’m really sorry please forgive me and my anxiety

If she was not detectable 24 or more days after your encounter, she could not have transmitted HIV to you. She would have become positive by then

Correct. If her test is negative, no need for you to test
2 - should I tell her to wait till 28 or there is no difference ?
No difference
3- if her tested negative at 24 or 27 and mine was negative 28 days post-exposure do I need further testing ?
Repetitive- the answer is not going to change
4 - I know the risk of frottge and kissing is zero but I’m afraid of penetration am I right ?
She said there was no penetration. I would believe her but the tests will also confirm that you did not get HIV.
You need to take a deep breath and relax. One follow-up remaining. EWH

Hello Doctor Hook,
My partner, a sex worker, sent me her test results: “HIV 1, 2 antigen + antibody,” it was a 4th generation test, correct? The results were negative, done on January 22, 2025, at 20:00. Our exposure was on December 27, so that’s approximately 26-27 days later. I assume her last exposure was December 26, 2024. She works in a brothel hotel, and I insisted on the test due to potential brief penetration.
I know this may be repetitive, but please bear with me:
1. is my risk now zero 100% , and can I move on without worry?
2. Is it normal for results to come out in 50 minutes?
3. Do I need to get tested after her results?
4. I was concerned about her being in the window period, but if she’s undetectable after 26-27 days, can she still transmit the virus to me?
Thank you in advance, Dr. Hook!

You are correct- this post is repetitive. The results reported are for a 4th generation test for HIV and prove that she could not have infected you during the encounter you have described. Final responses:
1. is my risk now zero 100% , and can I move on without worry?
Yes
2. Is it normal for results to come out in 50 minutes?
Yes
3. Do I need to get tested after her results?
No
4. I was concerned about her being in the window period, but if she’s undetectable after 26-27 days, can she still transmit the virus to me?
26-27 days is well beyond the "window" period when she could have infected you
This completes this thread. You should move on. There is no need for you to test and no need for continuing concern. There should be no need for you to return to the Forum regarding this thread. If you do, the question may be deleted without a response and without return of your posting fee. EWH
