[Question #12545] Blood exposure-3

Dear doctor, thanks for your previous assessment and advice. Hope you can pull my previous thread as this is related to the same exposure where I licked a deep fresh cut on the forehead of a CSW which had blood that went in my mouth. As I had earlier said that i got 3 positive 4th gen ag/ab tests positive after this exposure which really scared me. but the ab tests came negative and the RT PCR for HIV-1 also came negative. However, this RT PCR was for only for HIV -1. I am worried as the three positive 4th gen tests spooked me no end. Should I be retesting? My mother said I should not and as per experts overall opinion on this forum ( Dr. Hook), I decided not to test again and believe my results. U hope u am not making a blunder by not testing again. Moreover, the RTPCR was not for Hiv-2. Should i be worried about hiv-2?

Sorry, I forgot to add I am in India.my RTPCR was Quantitative RNa for hiv-1 only taken at 21 days.

One more query, is the hiv virus present in normal blood and menstrual blood the same or does normal blood in body( like coming out of a wound on forehead of infected person) contain more virus than menstrual blood. I ask this as i licked normal blood from the wound. Do you think I need additional testing for hiv-1? Also do you think I have chance of being infected by hiv-2 after reading my exposure as explained in older thread.should i get additional testing for hiv-2?

I'm sorry you felt the need to return to the forum. I'm afraid there is nothing more we ca offer you. While I understand how upsetting it was to have an an initially positive HIV test, the subsequent test results have proved it was a false result and there is no possibility you have HIV. Dr. Hook reviewed those results with you in your two previous discussions. To my knowledge, HIV2 is more common in India than in North America or Europe, but still causes a small minority of infections there. Your confirmatory antibody tests ruled out HIV2. And the most recently developed HIV RNA PCR tests include HIV2 as well as HIV1. You can double check with your doctor or clinic whether that was done in your case. It is increasingly available and testing for it probably can be arranged in India -- something to discuss with your doctors.
I am confident you do not have either HIV1 or HIV2. But all things considered, doctors with HIV experience in your country undoubtedly are more expert than we are in sorting out your concerns about HIV2. So we really cannot add anything beyond what I and Dr. Hook have advised. So we'll need to make this your last question about all this on our forum. Thank you for your understanding.

Thanks for your reply doc. The hiv specialists here are sometimes unaware therefore I rely on your advice. As you would have reviewed my previous threads, in your opinion was there any chance of transmission of hiv by licking a fresh wound? Dr hook was categorical that hiv is not transmitted by licking of blood. Also, i read that hiv-2 is more difficult to acquire. Would this be more true in my kind of exposure therefore making chances of hiv-2 transmission even less possible? Should I be even testing for my kind of exposure? Would you have? My test RT PCR RNA was only for hiv-1. Please reply. One more detail- 7 days after the result of RTPCR RNA, I got a POC ab test for hiv-1&2 ab in a lab by blood by vein. They do a rapid kit test. It also was negative. Does this mean i am safely in the clear for both hiv1&2. More importantly, in my kind of exposure, would you have even recommended testing? Please reply doctor.

I read your previous threads and agree 100% with everything Dr. Hook said, including absence of HIV risk from wound licking and the reliability of the test results you have had since the initial false positive results. I also agree you didn't need HIV testing in the first place, so to some extent your anxieties partly responsible for your anxieties about the false positive results. I've already said you don't need additional testing for HIV2; but I also said you should rely on your own doctors. (It is nonsense that HIV specialists in your country are likely to be "unaware" of proper HIV diagnosis and care, especially if you are in or near a major population center.)

If you remain concerned about any of this, I would advise you to go back and carefully re-read Dr. Hook's replies.

Thank you so much for your reply. I am just submitting this last query-
1. If the wound was actively bleeding and I ingested hiv contaminated blood directly from the wound, some of which remained in my mouth and some swallowed, would it still be a no risk situation for hiv? Can some cuts in my mouth/ tongue transmit hiv in my kind of case it of blood from wound in mouth?
2. Can i clear myself of any doubts about having got hiv 1 or hiv 2 in my facts and situation without any more tests?
3. Can I resume normal intimacy and unprotected intercourse with my wife?
Thanks to both of you for all your patience and expert replies. It has helped me a lot. You people have been most kind and accommodative to my doubts which were many times repetitive. Thanks a ton to both of you.

1. Still little or no risk. Oral exposure to HIV always is low risk. Even swallowing blood or semen from an infected person rarely results in catching the virus.
2. Yes. Clear yourself of all doubts.
3. Yes. You never needed to stop unprotected sex with your wife.
As I said above, if you continue to question the situation, re-read Dr. Hook's replies. And believe it! Whatever else, you may not ask any further questions about this situation. The forum does not permit repeated questions on the same topic or exposure and this is your third, with repeats of mostly the same questions.
