[Question #12574] STI Concern

Hey Doctor,
On 09/01/2025 I had an encounter with a CSW, had protected sex and oral sex as well as kissing all over the body (me giving) and french kissing. However at the very end I took off the condom to ejaculate in her mouth and she suddenly took my tip in her mouth for about one minute. I panicked and took 2 doses of doxycycline 100mg daily for 14 days starting 10/01/2025 but missed a dose on day 13 (which I took instead on day 15). I’m now worried about the possibility of gonorrhoea even though I have no symptoms (could that be due to the doxycycline dealing with the symptoms but not eradicating it?). I know taking antibiotics messes with the test results but I am going back home to Indonesia in two days where STI testing is highly stigmatised and hard to access. Should I be worried at all for Gonorrhoea? Or can I consider myself clear due to the lack of symptoms? Would you recommend I try to get tested anyway? Thank you.

Welcome to the Forum.  Thanks for your questions.  I'll be glad to comment.  The encounter you describe was low risk.  Most commercial sex workers do not have STIs and most single exposures do not lead to infection.  When they are, oral infections are less common than ano-genital infections.  Your exposure was largely condom protected and and the doxycycline you took has virtually eliminated any change of syphilis or chlamydia and reduced your risk for gonorrhea by about 50%.  If you had acquired gonorrhea, it is more likely than not that you would have developed symptoms.  

It is unlikely that you were not infected but not completely guaranteed.  Testing is always a personal choice and if you choose to test, it is more likely than not that the result will show that you were not infected.  Many of our clients choose testing in large part for the reassurance that a negative test provides.  I would put you into this category.  EWH

Thanks for that. So just to confirm, regarding the doxycycline, missing a dose does not matter in this case? Also, is it possible that the doxycycline merely kept off the gonorrhoea symptoms and if I hypothetically had caught it then symptoms would appear in the following days since I’ve finished taking the antibiotics? Or do the antibiotics not work that way and that since no gonorrhoea symptoms appeared in the period I was taking the antibiotics then I can assume that I had likely not caught it (since doxycycline is only reliably effective for chlamydia and syphilis)? I’d also like to add that she had what looked like back acne but I’m wondering if it was actually a skin condition from an STI. When she took off the condom she also gave me a handjob with the same hand she used to give me a handjob with a condom on, which means it came into contact with her vaginal fluids. Finally since testing is quite hard to do in my country do you think it would be good to take something like cefixime or azithromycin to eliminate the risk for gonorrhoea? Thank you

This is repetitive.  Missing a dose does not matter.  Doxycycline would still be in your system despite a missed dose.  

Most (over 90%) gonorrhea is symptomatic.  Given your anxiety, if you had gonorrhea you would have noticed.  

I advice against taking additional antibiotics.  I do not see the need.

Yes I understand. Thank you again for the prompt reply. Given what you’ve just explained to me, I take it that if you were in my position, discounting the unnecessary anxiety regarding STIs, you would not bother with any further testing and continue having unprotected sex with my regular partner? Thanks again.
