[Question #12595] Possible HIV Exposure

Good evening,
I am asking this question for a friend, so my friend may had an HIV exposure he went to go see asian massage parlor in Dallas Texas. So he had vaginal sex but he used protection but 4mins in the noticed the condom fully slipped off,  he doesn't remember ejaculations. He started PEP under 48hrs.

He wants to know  is his risk a lower exposure?

Starting PEP under 48hrs will it be effective?

And he plans getting a full STD panel 3 months after this exposure with no other risky activities.

Welcome back to the Forum.  In our experience clients who claim to be asking questions for a "friend" are actually asking for themselves and are embarrassed to admit it.  This pretense impairs the quality of our interaction with persons with questions.  

This was not a particularly high risk exposure.  The condom slipped off leading to a single unprotected sexual exposure.  It is statistically unlikely that your partner had HIV and even if she did have untreated HIV, your risk for infections would be less than in in 2000, i.e. chances are more than 99.9% that you were not infected if she did have HIV.  If you had been in my clinic, I would have strongly recommended against PEP.

When persons need PEP, the sooner the  medication is taken, the more effective it is.  PEP started 48 hours is slightly less effective than PEP started at 24 hours of less.  In terms of testing for STIs:

1.  You can test for gonorrhea and chlamydia using a urine test any time more than 3-5 days following your exposure.
2.  Testing for syphilis will be almost completely conclusive at 30 days and completely conclusive at 6 weeks
3.  You can test to prove that you do not have HIV any time more than 30 days after completion of PEP.  If you had not taken PEP, you could have gotten conclusive results much sooner.

Yeah I am embarrassed about it, I just never had a condom failed on me. I was nervous but the escort claims she is disease free and she test negative HIV Jan 02 2025 she didn't show me the paper work, but she claims she tested every 3 months

Thanks for the additional information.  No reason for embarrassment.  Condoms fail about 1% of the time.  I congratulate you on your safe sex practices.  EWH---

The decision to continue PEP or not is up to you but in my opinion, there was little reason to start it.
