Hello doctors, I am from Sydney Australia. i recently went to a body slide massage just 3 days back. After the massage the masseuse stimulated my penis with her breasts and then sat over my abdomen in a cow girl position and held my penis with her hand to the back and started stroking my penis and then she bent over my body and she let go off my penis and started moving back and forth with her vagina rubbing my penis we were completely covered in oil. I did not feel any penetration, I hope the extra massage oil might not have realised me of the penetration. This might have happened for 5 minutes. After that I requested get down and give me a handjob. How worried I should be for HIV.

I asked the masseuse if there was any penetration she said no. I am very concerned and do ARS symptoms occur in 3 days and also the massage place is a known erotic massage establishment and  I called NSW sexual health infoline. and they advised no concern at all. But your expert advice is greatly needed.

Welcome back to the Forum although I’m sorry you felt the need. This is your 3rd, now repetitive question regarding a no risk exposure.  No penetration, no risk.  You felt no penetration and your partner told you there was no penetration.  There is no need for concern and no need for testing.  You do however need to address your anxiety over your casual contacts.  If you cannot do this yourself, I suggest you seek confidential input from a professional counselor.  EWH---

Thank you for your answer doctor. Sorry for coming back but this is a new incident which happened 3 days ago. 

It is not following up on my previous questions from December. For that incident, Both the 3 week hiv ag/ab test and 45 day hiv ag/ab test came back negative. 

Do you reckon for this recent incident I do not need a hiv test?

I see no medical reason for testing.

Thank you so much Dr Hook. This is an entirely new incident as I mentioned. This happened on 01/02/2025. Today 07/02/2025. I am having profound night sweats, coughing, muscle aches but my temperature is under 37 c. I am feeling very worried. I  now think I should have been careful from my previous incident that I asked questions to you in December as all the tests were negative for that incident . I feel I acted stupidly and I think there might have been penetration even though the masseuse said no. I always go thinking of getting massage and hand job that’s it. I did not ask for any sex like that  Sorry you might think this is panic but I was feeling fine until yesterday. Can I go for a Hiv viral load test at 11 day and get tested or should I go for hiv combo at 21 day as initial testing. Please doctor your advise will be greatly appreciated.

Please do not delete my follow ups thinking that I am panicked.
I am also worried as I didn’t experience any kind of symptoms for my last incident in December until 18 days which were mostly hot flushes but now I am getting symptoms at 6th  or 7 th day and have night sweats, joint pain, dry cough that only stopped after I had hot water. Also I am worried for my wife who is currently in overseas and she will be back next week and will definitely want to have sex. 
I definitely can say that in this incident I ejaculated when she gave me handjob after getting of my abdomen but not when she was sliding over my penis. I call Sydney SHC’s they all advised that they cannot see me has it is very low risk and asked me to go to go and request a full sti check.
1) are my symptoms onset of ARS?
2) if penetration occurred how risky episode is this one?
3)Hiv viral load in Australia is not covered by Medicare so all GP ask us to go for HIV combo? Should I get a combo test?

That it is a new event does not change it's similarity to your other questions.  You are worrying unnecessarily and manufacturing risk and most probably penetration that does not exist.  

Cough, hot flashes and night sweats are not signs of recently acquired HIV.  As a final response to your last follow-ups:

1) are my symptoms onset of ARS?
See above- they are not.

2) if penetration occurred how risky episode is this one?
Most commercial sex workers do not have HIV and even if penetration occurred and she had untreated HIV, the risk for infection would be less than 1 infection for every more than 2000 exposures, on average,

A negative combo test would PROVE that your symptoms are not due to HIV (you have been told this in the past).  A negative HIV RNA PCR test any time more than 11 days after an exposure would provide conclusive evidence that you did not acquire HIV and no further testing would be needed even in the unlikely situation that you were exposed.
