[Question #12645] Dysuria Without Discharge

Hello,
I have a question about a symptom I have had approximately 2 days after receiving unprotected oral from another male from an app dating website. I have abstained from all previous activities and topping, etc.

2 days after I developed some Dysuria, but it happened after I had ofcourse once again googled and convinced myself I had Gono. I have not gone to get tested as I won’t be able until Tuesday next week. But I have had zero discharge (I have been checking a lot and touching the head), maybe a little inflammation at the tip, but I have that burning sensation when peeing and checking for about 5 days now.

My question is can you have Gono if you only have the Dysuria and no discharge after 5 days post exposure? Or would discharge be present and known if I had try Dysuria. It is hard to tell from internet and such as it just has the symptom listed. I know that my anxiety also gets to me and can that cause that burning sensation because I am anticipating the symptom?

Welcome back. Thank you for your continued confidence in the forum.

Almost certainly your gonorrhea test will be negative. As it happens, my colleague Dr. Hook was the lead researcher on probably the most relevant published study. It showed conclusively that dysuria alone, without discharge, rarely is due to gonorrhea or any other STI. Another possibility is a viral infection, specifically adenovirus. Adenovirus is primarily a respiratory virus, causing common colds. conjunctivitis (pink eye), etc -- but some types of the virus (especially adenovirus type 19) can cause urethritis, acquired by oral sex. Visible inflammation at the tip of the penis often is present. From your description, I doubt your symptoms are primarily anxiety related.

Herpes due to HSV1 also is possible, if you are not in the 50% of the population with prior infection, usually in childhood. However, herpes almost always causes external blisters and sores, not only urethritis.

There is no treatment for adenovirus, but it clears up on its own in a few days. At this point, my advice is to wait for your gonorrhea culture result, with no other tests or treatment in the meantime. However, if the symptoms worse and/or you develop external blisters or sores, they should be tested for HSV. But most likely the problem will fade and you may never know the exact cause. But probably nothing serious.

Thank you, specific to adenovirus, is there any risks about sexual activity if that is the case and I have sex while this is possibly the cause? Interestingly enough, the burning has gone/subsided over night and such and I have calmed myself to be honest and not so focused on it any more. Still no discharge or sores or anything and the person told me they did not have Herpes so my assumption is they would know if they did.

I would strongly advise holding off on sex with anyone -- new or ongoing partners -- a few more days. We don't know how long it might take for adenovirus (or other possible respiratory viruses acquired by oral sex) might be transmissible. But I would say you're good to go 7-8 days after onset of the dysuria and maybe 2-3 days after symptom resolution.

It is untrue that people with oral HSV infections would know if they have it. Half or more do not know. I still have a strong suspicion you have herpes, especially with the dysuria now resolving (herpes would last a lot longer). But still the fact of asymptomatic herpes is something all sexually active people should know.

Looking at your last comment, you said you have a strong suspicion I have herpes? Is that true or taken a bit out of context because you said it would last longer if I had heroes? I had dysuria for a little less than 2 days and it’s gone now, like I said I haven’t been checking constantly like I originally was and still no discharge, etc. I have had this burning symptom before when I have been anxious about previous encounter and when I took the tests all was negative.

Oops -- a typo! I left out the most important word:  it should read "I still have a strong suspicion you do NOT have herpes." So sorry. (Even better wording would be "I remain confident this isn't herpes, especially with the dysuria now resolving....)

Thanks for clarifying, but now this gets my mind racing a bit of the possibility of herpes or asymptomatic herpes. Does dysuria always happen before getting herpes sores with herpes or should it last longer and until visible sores/lesions? I read that the dysuria from herpes is from the pee burning against the sores/lesions if around the urethra? Is that true or dysuria is just a standalone symptom whether sores are there or not? Sorry this got me a bit worked up and worried about herpes, checking for bumps, etc and see things I probably would normally not pay attention to. Thanks.

And I guess one other follow up, could giving oral sex cause tonsillitis from herpes? I say this because the guy who gave me the oral said he has this. He’s being tested as well.

It is exceedingly unlkely you have herpes; it rarely causes urethritis and urinary pain without visible external herpes lesions. In other words, dysuria only rarely would be a "standalone" symptom. I've never heard of initial oral herpes causing tonsillitis, especially in the absence of multiple sores in or around the mouth. Almost certainly your partner's tonsillitis has nothing to do with giving you oral sex.

You really should let the herpes business go without further worry about it. I'm sorry I even mentioned it. But if you develop more symptoms to suggest herpes, or if you have other questions about it, feel free to ask a new question and choose the herpes category.
