Hello Dr Hunter. İ had anal sex 1 day ago. İt was protected but the condom broke and ( unprotected  1 minute) . I am bottom.  I can't reach my partner . Do İ need to take pep? Or should i wait and have a PCR test after 10 days?  Some sites say that a negative PCR hiv test is definitive after 10 days. I trust your information. I am scared.

Welcome to the Forum and thanks for your questions.  I’m sorry to hear of your condom mishap.  Breakage occurs about 1% of the time and is slightly more common with rectal sex.  If your partner had untreated HIV your risk of requiring infection is about one in 100-200.  You are in a better place to judge the risk than I, considerings things such as did you ask him about his status or whether he was taking PrEP or not, etc.

PEPis a personal choice. Your risk on infection is somewhere in the neighborhood of one percent. If you cannot find out more about your partner in the next day and don’t know much about him, you may choose to take a PEP. The sooner the better and there is no use in getting it more than 72 hours after an exposure.

If you choose not to take pep, you can determine whether you were infected or not with an HIVRNAPCR test anytime more than 11 days after the encounter. If you decide to take PEP you will need to wait until at least 30 days following completion of the medication to get a conclusive test result.

Hello doctor, I had an hiv RNA PCR test 15 days later. The result will be announced after 2 days. If the result is negative, is it enough? Also, is 4 and 6 week antigen and antibody testing required? - What is the accuracy percentage of HIV PCR RNA amount on day 15? - What are the chances that the Week 4 antigen antibody test will change? -I have sores on my lips and inside my mouth. I don't know if I have HPV or not. Should I take another HIV test on day 28? What do you suggest me? Thank you.

If you decided to not take PEP, then your PCR results are conclusive and no further testing is needed. Your test results will not change.  EWH---

Hello doctor. My results came out today. Day 15 hiv RNA PCR: Not detected. (negative). Some doctors wrote that the 4th week antibody antigen test is necessary. I hope the result does not change. You said there was no need to retest. I trust you. Thank you.

My Hiv Rna PCR results: Day 15 hiv RNA PCR: Not detected. (negative) Some doctors wrote that in addition to the PCR test, the 4th week antibody antigen test is also necessary. But you wrote that there is no need for another test. I trust you.

Your PCR test result is conclusive. In the past some experts, including ourselves, have recommended a follow up fourth generation test but experience has shown that that is no longer necessary. Please accept your test results is conclusive. No further testing is needed. I encourage you to move forward without concern.
