[Question #12660] 4th gen test + Oraquick

Asking for friend whose English is not good.
Male with exposure with girl in Dubai who is stripper in Vegas.Exposure was due to potential condom slippage in last 2 mins. She seemed clean and asked me if I was clean.
Day 23-27 had headache, high fever, cough with phlegm, chills, numb toe. Tested 4th gen ag/ab on day 28 (after 27.5 days). Result negative.
Day 29-35, had chills, cold feet and was on antibiotics.
Some rash on chest and a mouth sore on day 42-46. Tested Oraquick oral swab day 49 negative. Day 60 did Oraquick again negative.
1) should I continue testing? Am I safe?
2) Was the 4th gen test too close to symptoms/sickness? Should I have done another one?
3) do the symptoms before and after show that I was infected? I was super stripped between day 24 and 34 so it might have impacted my body. Thanks

Welcome to our Forum. Thanks for your questions.  I'll be glad to comment.  The exposure you describe was low risk.  I suspect the reason that your recent partner asked you about your status was because she does not have HIV.  Certainly most commercial sex workers do not.  Even is the very unlikely circumstance that she had untreated HIV, your risk of infection would have been less than 1 infection per more than 2000 exposures, on average.  Your recent OraQuick results should be believed.  Your symptoms were not due to HIV(negative tests in  the presence or after symptoms are always positive).  In response to your specific questions:

1.  There is no need for further testing. It's time to move forward without concern..  you are safe.
2.  See above.  No need for further testing of any sort.
3.  Repetitive.  The answers will not change.  Stress will not mask a positive test if you were infected.

I hope this information is helpful.  Please don't worry.  EWH

Thanks Dr for the response. A couple of follow ups.
1) Oraquick could be prone to potential human errors, do you think without considering two Oraquick tests, is 4th gen Ag/Ab test on day 28 (after 27.5 days) still a good test to rule out infection?
2) The chance is 1 in 2000 but in your research have you come across infection cases from female to male in a single exposure? Thanks

Sigh.  Your unwarranted paranoia is showing

1.  Yes.  At 28 days a negative 4th generation test would detect 98-99% of recent infections
2. Of course there are persons who acquired HIV following a single exposure.  That’s the way mathematical probabilities work.

You have one follow up remaining.  EWH
