[Question #12671] HIV Risk - Multiple Cuts/ tears on multiple fingers and fingering CSW

Hello Doctors,
Thank you for having this platform. It is really helpful for people like us to get our questions answered by experts in the field. Coming to the issue at hand:

I have multiple cuts on multiple fingers due to cold weather and frequent washing. Yesterday 10th Feb 2025 at around 8:45 PM I had a sexual encounter with a masseuse. I got a body-slide and hand-job from her. She put oil on her body. During the slide, I rubbed her genitals (vagina) with my hands. I never inserted one particular finger in, just rubbed the surface with my palm, mostly my fingers might've touched too. It was indeed wet with what I believe was a mixture of the oil and vaginal fluids. After we were done, She sprayed sanitizer/ alcohol on my hands after which I also washed my hands twice with soap thoroughly.

On the way back, I realized I have cuts on my fingers due to the cold weather that had not heeled (could see some old blood). The best I can describe is the cuts are looking red (which might be dried/ old blood) Obviously, I was tensed and couldn't sleep the entire night. So in the morning at around 8 am I went to the ER and explained the attending physician my encounter. Whilst they said my encounter was low risk, they were willing to prescribe me PEP due to my fear and anxiety. So at 11:30 am or approximately 16 hours after the encounter I had my first dose of PEP truvada + tivicay. I intend to diligently complete the 28 day course.

Following are my questions for you:
1) How risky was my encounter?
2) What are the risks that lie with this encounter?
3) I am visiting family and there is a newborn boy, what precautions do I need to take around the baby and family? (Especially when I still have that dry skin and cuts on my fingers)
4) Will the side-effects be super heavy that my family might feel that something is wrong?

Welcome to the Forum.  Thanks for your questions.  The idea that there is meaningful risk for acquiring HIV through contact of your hands and fingers with infected genital secretions is an incorrect, internet based myth.  I am unaware of any proven cases of HIV acquired in the sort of exposure you describe even in the unlikely circumstance that your masseuse had HIV.  Taking PEP for such an exposure is unnecessary, a waste of resources, and prolongs the time it will take to prove that that you were not infected if you chose to test.  In response to your questions:

1.  See above.  This was a virtually no risk event.  The presence of fresh cuts, scrapes and cracks on your fingers and hands do not change that.
2.  Repetitive. See above.
3. No
4.  Typically the side effects of PEP are mild and not noticeable to others.

Thank you Dr Hook for your reply. It gives me some reassurance that the risk is very minimal if not non-existent! 

I will continue with the PEP as I have taken it for two days now.

Hello Doctors,

I am diligently taking my pep at 11:30 for the last 12 days. However, since yesterday, I have come down with a severe sore throat and muscle aches . I have booked an appointment with my primary care physician for Monday but I’m worried this is either a severe side effect or hiv seroconversion!

You were not at risk to start with and even if there had been a meaningful exposure, your risk for infection in PEP is minuscule.  I am confident your symptoms are coincidental and unrelated to the no risk encounter that you described.
